What do you mean by hyperlipidemia?

During diabetes when there is an increase in the concentration of lipids or fats in the blood. Then we call this condition hyperlipidemia. When they are at a marginal increase and are not affecting us clinically, we call this condition mild hyperlipidemia. 

Mild Hyperlipidemia | What Causes High Cholesterol?

But technically, mild hyperlipidemia doesn’t bring that much impact on the management. So when it is mild, like we do in diabetes, we recommend lifestyle modification as it helps. 

But if the hyperlipidemia is more than normal parameters, we prescribe some medicines. Often people enquire that they have diabetes because of hyperlipidemia. But I want to clarify that diabetes can cause hyperlipidemia; there can be an increase in cholesterol which I will discuss further, but high cholesterol doesn’t cause diabetes.

Why is there an increase in cholesterol or fats during diabetes? 

First, it increases because, in the initial stage, the patient develops insulin resistance, one of the initial modes of diabetes. Due to insulin resistance, the body stops responding to the insulin. As a result, to overcome the scarcity of insulin, our body secretes more insulin. 

Therefore the liver, as a response, produces more fat cells, which gradually increase the concentration of fatty acids in the body. The second reason is the release of fatty acids from our adipose tissue or fat cells toward the liver. This occurs when our body is low on insulin. The third thing, mainly our insulin, prevents the conversion of our triglycerides into the fatty liver. So when the insulin level drops, there is an increase in the level of fatty acids.

The four mechanisms | What Causes High Cholesterol?

However, we can go more technical in these three or four mechanisms: the chemical reaction occurring in the liver or the proportion of the insulin, which finally levels up the fatty acids in the body. So now we will talk about three essential elements in lipid profile. 

First, any diabetic patient’s total cholesterol should usually be less than 200. Next, our three most critical subparts, one is triglycerides. Our body triglycerides should be less than 150; we target it to be less than 150.

Another thing is we expect to have an LDL of less than 100. Another and rather only one thing that we say that should increase, and we ask the patients to increase is good cholesterol or HDL. It should be more than 60. 

Role of HDL

However, why should it be above 60; what benefit does HDL provide to the body? HDL is anti-inflammatory and antioxidant, and it helps in reducing cholesterol stress or basic fat. It is the healthy cholesterol in our body.

What are the measures to increase HDL in the body?

There is only one way to increase HDL in the body by exercising. Exercise is the only way to increase HDL in the body. Now let’s come to the next: why are we putting so much stress on hyperlipidemia in diabetes. The reason is that if the concentration of lipids in diabetes is high, they are at more risk of heart attack or heart-related diseases. 

It is twice in males and thrice to four times in females. That is why it is important to control the concentration of lipids in diabetic patients to prevent the risk of cardiac problems.

What are the Risk Factors of  Hyperlipidemia? What leads to hyperlipidemia?

The main reasons that overlap is not doing any physical exercise, consumption of fatty food, intake of excessive alcohol and smoking, improper diet, and stressful life; theoretically, people are predisposed to this, so these are a few factors that may lead to hyperlipidemia in patients.

So when these patients come for treatment initially, we suggest walking for 30-40 minutes; we give the same advice to hyperlipidemia patients that you must use the body’s stored fat. So we advise them to do muscle strengthening and exercise, which burns energy. Avoid trans fat in the diet, regulate the concentration of fat in the body, and stay stress-free as much as possible. 

Avoid alcohol and smoking

What is Metabolic Syndrome? 

We use the word “metabolic syndrome” very often these days. It is also known as syndrome X and it was named after the term coined in 1986. 

It is not a disease; instead, it is the constellation of reasons that means its complex of disease-causing things, including blood pressure, sugar, cholesterol, and obesity. 

What is Metabolic Syndrome comprised of? 

All these things in combination come in metabolic syndrome, and out of these five, if three things are present, we confirm this condition. If we want to specify, if we are talking about obesity, then it is central obesity, especially abdominal or stomach obesity, which correlates with heart diseases. 

The second thing is cholesterol, in which we see triglycerides and HDL. The fourth is sugar or fasting blood sugar, and the last is blood pressure. These five things together make up this syndrome. 

And if out of these five, three things are present simultaneously, then we call that condition metabolic syndrome. It is so termed because those with this syndrome are at higher risk of heart diseases like heart attacks or strokes. 

Risk factors 

And we have a reason to prevent it, and the patients are also aware of the term metabolic syndrome its the risk factors. 

Lack of physical activity 

We usually see Metabolic syndrome in people with less physical activity, those who consume carbohydrates-rich food, those who have a sweet tooth, or eat a lot of refined or fried food. They are at higher risk of having this syndrome. 

Age 

Apart from this, there is the age factor. With the advance in age, the risk of having this problem also increases. 

Steroids 

Certain steroid-related medicines also increase the risk of this syndrome.

Like steroids prescribed during COVID may pose the risk of having sugar or obesity. 

How to prevent Metabolic Syndrome 

Physical activity 

Daily exercise or walking at least 30-60 minutes per day is the most important measure to prevent this condition. We can quantify this by taking 10,000 steps or more than that. 

Healthy diet 

Second, we must have a healthy lifestyle in the form of healthy fruits and green vegetables. Avoid refined food items. Apart from this, if there is an increased level of other this syndrome parameters like sugar, blood pressure, and cholesterol. 

Bottom Line 

So by keeping these things under control, we can reduce the risk of heart-related diseases and strokes. As I told you, Metabolic syndrome is a complex condition, and we can both prevent and treat it. You can consult your doctor and take the necessary treatment and more information and treatment.

What is Urine Incontinence? 

Urine Incontinence means unwantedly leakage of urine (stress urinary incontinence). With the advancement of age, many females have to face this problem. When they cough, sneeze or jump or when they go outside their home, they experience leakage of urine. 

Reasons for Urinary Incontinence(पेशाब न रोक पाना)

There are many reasons associated with this. The passage of the urine in females is very small, it is about 4 cm long. And is located above the vagina. The vagina is the birth passage. During pregnancy and childbirth, the lower area that is the urethra becomes loose. Due to this the urine pipe that is the urethra loses its support. 

The pelvic muscles also get loose or weak due to which when females cough or laugh, they pass urine involuntarily.

So in this case what happens is, one wants to go for urine after every 5 to 10 minutes. So bladder training is also very important. And make sure that you don’t go for urine before 2 hours. So after passing urine please count to 10 and give another push so that the remaining urine flushes out. Then you can use tissue paper  to dry yourself and avoid the smell of urine. We don’t have to wash our lower part, again and again, it may increase the risk of fungal infection.

Today the Dr. Gaurav will be talking about this common problem you all are facing. Weight loss or gain can occur suddenly or because of some significant underlying symptoms of thyroid-related issues. 

Thyroid and Weight

The thyroid is a gland present in the neck, and its primary function is to control the body’s metabolism. This gland’s other functions include maintaining other body activities, heart rate and control weight of the body. If its function is imbalanced, it leads to a sudden increase or loss of weight. 

What is Hypothyroidism? 

So if there is a condition of hyperthyroidism in which the thyroid gland over-functions, it results in an increase in metabolism, more commonly known as basal metabolic rate. So hypothyroidism may lead to sudden weight loss. However, we can’t confirm the rate at which there will be weight loss. But it usually varies between 5-10%, but in exceptional cases, this value can be more.

What is Hypothyroidism? 

Similarly, we have another condition of the thyroid gland: hypothyroidism, in which there is a decrease in the function of the thyroid gland. This condition results in weight gain. Average weight gain is about 5-10 %, but it majorly varies on disease severity. If it is mild, then there is not much gain in weight; however, it may cause slight swelling on the face. 

Thyroid problems in women

Thyroid-related problems usually commence with the symptoms like swelling, weight gain, lethargy, feeling sleepy all the time, and one may feel tired while working. But to diagnose this condition, a blood test is done. 

After treating this condition, the body may come back to its original form, i.e., weight gain is lost and vice versa. Like in hyperthyroidism, weight loss is gained back. Similarly, in hypothyroidism, the weight gain is lost after treatment.

But it is not like the entire weight gain is lost, but yes, there is a decrease in weight. So today, I discussed thyroid-related problems: the possible reason for sudden weight gain or loss in the body.

What is an adrenal gland? 

The adrenal gland is a small-sized gland situated over both kidneys. They weigh approximately 3-5 gms. They are a very important gland since it produces steroid hormones that control our body’s blood pressure and electrolytes. 

What causes adrenal gland disorder?

So diseases associated with the adrenal glands are related to an imbalance in the production of hormones. Scanty hormone secretion may rise to situations like low blood pressure and disturbance in electrolytes like sodium and potassium. If the adrenal gland works less or there is any deficiency, it could lead to such problems. 

What is Addison’s disease?

Usually, this condition is called Addison’s disease. Addison’s disease is one of the diseases associated with the adrenal gland. The second disease as mentioned earlier, a rise in blood pressure, may also be attributed to the adrenal gland. 

In this, one section of the adrenal gland is responsible for the production of hormones that controls blood pressure. 

What is Conn’s syndrome or Hyperaldosteronism?

If the adrenal gland produces a hormone in excess amounts, it suddenly raises blood pressure. So in a situation where we cannot control blood pressure even with many medicines, or we can manage it after giving many drugs, we suspect that the adrenal gland might be causing this problem. Then we think of getting tested for adrenal disorders. This condition is usually known as Conn’s syndrome or Hyperaldosteronism. 

This condition arises when there is a hyperfunction of the adrenal gland. Its diagnosis requires a particular test that further confirms whether there is a problem in the adrenal gland or not. 

Problems associated with adrenal gland

Apart from these, there can be other problems associated with adrenal glands like tumors or cancer, which usually get diagnosed when we do an ultrasound to check for other diseases like stomach pain or any other related problem. 

Diagnosis of adrenal gland disorders

So its diagnosis requires a specific test. To treat adrenal gland-associated diseases, if the hormone production is less, we can supplement the hormone outside and maintain its concentration inside the body. 

And if there is overproduction of hormones, their treatment will depend upon the type of disease like a tumor or anything else. Likewise, according to the kind of disease, we plan the treatment. 

So if you have a similar problem like there is a rise in blood pressure or it is difficult to control it or if blood pressure is increasing at a very young age like at the mere age of 30 you have a sudden rise of Blood pressure, or it is coming very low, you have symptoms of vomiting. There might be a suspected condition of the adrenal gland. 

So under these circumstances, you must visit your doctor for adrenal evaluation.

The purpose of anti-ageing 

Human beings are born to look beautiful, and they deserve to be beautiful. And it stands for both male and female today. With the aid of Medical Technology, our life has been prolonged. At the time of independence, the average lifespan was between 27 to 35 years. 

But if you think now in today’s age group, we expect that if you have taken care of yourself and you are a healthy person, you will live for at least 80 to 90 years. But our body doesn’t remain at the same stage as we were in the age group of 20 to 25 years. Our body undergoes many changes.

Because protien in our body starts breaking down due to aging. Elastin and collagen fibers, which are protein, start breaking, resulting in wrinkles. So we need to increase our protein intake in any way. If you are a vegetarian, you can have nutrela, cotton cheese, soya bean, or pulses.

What should be done? 

Children often have this tendency to put anything in their ear, nose, or mouth. Today we will talk about this condition where the child unknowingly inserts anything in their nose. If we look at the history or the old data, then we will know there are 4-5 things. That the children often insert into their noses. 

Which objects to keep away? 

The first and foremost thing is a rubber or eraser that children often use. And sometimes children put a pencil with rubber attached to their nose. And that rubber detaches from the pencil and stays in the nose. Another common foreign body is nuts if the children have nuts, chicken peas, or peanuts. They put pieces of them in their noses. Other things like plastic or paper or small pieces of foil are a few common things children insert in their noses while playing. While playing with beads, few children put them on their noses like beads of necklace or bangles. Among all, the most dangerous thing that we often see is the children inserting small button cells into their noses.

Why do they do this? 

Children find button cells very interesting, and while playing, they insert the cell into their noses. Out of all mentioned things, the button cell is the most dangerous foreign object. 

What can happen? 

Now we will see how children present this condition. Toddlers can’t convey this; hence the alert parents can identify this situation that some foreign object is inside their children’s noses. 

How to identify a foreign body in nose? 

Blockage on one side of the nose may indicate the presence of foreign things in the nose. If both the nostrils are closed, there could be a cold, allergy, or infection. But if there is a consistent blockage in one side of the nose. Most of the time it’s closed, like in the morning, afternoon, and night. Then the parents must be alert that there might be a possibility that the child has put some foreign object in their nose. 

What happens if it goes unidentified?

Often we can see foreign objects in the nose. During the initial time with the help of a torch, the caretakers may see this object in the initial phase. But if the foreign object is inside the nose for 10-15 days and it is present as undetected. Neither the children told their parents. Nor did the parents notice that we often see the following type of presentation under these conditions. 

There can be a discharge of pus, bleeding from one side of the nose, and there can be a smell too. These children, if noticed closely, may have a smelly nose with pus discharge and bleeding. 

Also, in a few cases, a substance like pus, watery discharge, or there is already blockage of the nose, but it may cause bleeding too. If children show these symptoms on one side of the nose, then there is a high chance of a foreign body in the nose.

Bleeding in pregnancy is a very common condition. Almost 20 % of pregnancies are associated with bleeding. Statistically, 2 out of 10 result in bleeding during pregnancy. 

Types Of Bleeding During Pregnancy

We will discuss the types of bleeding during pregnancy in detail. 

Implantation Bleeding | How to Stop Bleeding During Pregnancy

First is implantation bleeding. This type of bleeding occurs after fertilisation; the embryo implants itself towards the inner lining of the uterus, i.e., on the endometrium. This implantation may result in spotting, but this bleeding usually occurs before the missed period. Precisely this bleeding occurs within 6-7 days of ovulation. And usually, it lasts for 1 -2 days maximum, it is light pink, and this does not require any treatment. 

Usually, we advise our patients to go for a pregnancy test whenever they miss their periods. If the test is positive, we can carry out a normal pregnancy. If the pregnancy test is positive and then it is bleeding, then we need to be cautious. Under these circumstances, we must consult our gynecologist as it could be an early sign of miscarriage. 

Threatened abortion 

Often the fetus may get separated from the endometrium a little, and all other parameters of the fetus are normal. Then this condition is called a threatened abortion. There is separation but not to that extent, so we give progesterone injection to support the implantation and advise rest. 

Signs of miscarriage 

Eventually, it gets settled. Apart from this, sometimes pateitns may have heavy bleeding, or there are clots, or they are having cramps, so these could be a sign of miscarriage. Still, we need to check whether the foetus is adequately expelled from the uterus after separation or not. Is it incomplete separation or complete? If it is incomplete separation, it may result in heavy bleeding, ultimately causing infection in the uterus. 

So for this type of bleeding, we must consult our doctor. Usually, whenever you are heavy bleeding accompanied by clots and intense cramps, it gives an idea of miscarriage. 

Subchorionic Hematoma

Apart from this, one other thing you must have read in ultrasound is a subchorionic hematoma. In subchorionic hematoma, there is blood pooling, resulting in clots between the foetus and the inner lining of the uterus. If the clot is small in size, we try to treat it with medicines and progesterone injection for support, and gradually it settles. But if the size of the clot is large or there is a big size hematoma, then there is an increased chance of miscarriages. 

Ectopic Pregnancy 

Apart from this, there is another important condition called an ectopic pregnancy. 2 out of 100 pregnancies could be in the fallopian tube, ovary, or any other place but not the uterus. We call this condition ectopic pregnancy. This pregnancy test would come positive, but the patient may have bleeding, intense pain, and dizziness. 

If you observe any of these signs, then it is an emergency condition because the diameter or lumen of the tube is tiny; it is self-evident the foetus can’t grow in tubes. In severe cases of pregnancy, there is a rupture of tubes resulting in the pooling of blood in tubes leading to life-threatening conditions. Under these circumstances, we must consult our doctor and get treatment accordingly.

Molar Pregnancy 

 Another condition is a molar pregnancy. This type of pregnancy is very rare, but the tissues present in the placenta wall are overgrown. Dense tissues in the placenta wall might result in bleeding, and sometimes bleeding may occur even after the DNC. There is recurrent bleeding after repeated DNC., so we suspect molar pregnancy under these circumstances. We need to discontinue the pregnancy as there is repeated growth of molar or abnormal tissues. 

Other Reasons For Pregnancy Bleeding 

Lastly, there are a few reasons, like infection in the cervix, which is the mouth of the uterus, which might result in bleeding. For this, we prescribe medicine and antibiotics to settle this bleeding. The most important thing that I want to convey is that if you are bleeding during pregnancy, you must consult your gynecologist, get your ultrasound, take medicine, and don’t ignore this condition.

What it is Anovulation?

Anovulation is a condition when ovulation does not take place. Symptoms of Ovulation (ओवुलेशन होने के लक्षण) During the normal process, inside our brain is the hypothalamus which secretes Gonadotropin-releasing hormones. These hormones act on the pituitary gland and release other such hormones responsible for the formation of follicles during the first ten days of the cycle after menses inside the ovaries. Then after some time, these follicles mature. 

Later,  after maturation, one follicle matures, and in the later stages, the egg ruptures. This process is called ovulation. Before ovulation, the secretion of luteinizing hormone increases significantly, leading to ovulation. After ovulation, there is an increase in progesterone concentration in the body. If by any means this process of ovulation is hampered. 

Reasons associated with Anovulation

There can be many reasons associated with this. There can be problems in the pituitary gland, some issues related to the hypothalamus, or an imbalance of hormones. All these hormones present in the body, like thyroid hormone, prolactin hormone, FSH hormone luteinizing hormone, are interrelated. If there is an imbalance in these hormones, like the pituitary gland dysfunction, then there is a delay of cycles, or the cycle is irregular or happens for fewer days. 

When there is a complete failure of the pituitary gland, like in some cases, the menstrual cycle does not occur, or they start and eventually stop after six months; we call this secondary amenorrhea. Ovulation failure is very common in these patients.

Symptoms Of Ovulation Failure

The patient often visits OPD with two main symptoms of ovulation failure. 

Irregular periods Symptoms of Ovulation (ओवुलेशन होने के लक्षण)

The first is irregular periods that we recently talked about, and the second is they come with infertility. Infertility means they are unable to conceive. Often patients come to the OPD and tell that they have been trying for an extended period, but they cannot conceive. So this state is a chronic anovulation phase in which ovulation is not occurring. 

Usually, we see that two or three cycles of ovulation are not happening in one year, and it may start again. So we call this condition an irregular cycle. But if there is chronic anovulation or completely no ovulation, this becomes one of the causes of infertility. 

PCOS 

Apart from hormonal imbalance, PCOD is a condition in which anovulation is very common. In PCOD, there is weight gain, facial hair, especially hair close to the jawline, onset of face acne, and an irregularity in periods. 

Endometriosis 

Apart from this, if you have been diagnosed with endometriosis, the quality of the egg deteriorates to a greater extent, and there is a complete disturbance in the ovulation pattern. 

Other Factors 

In some cases, excessive exercise may also disturb the pattern of ovulation. Secondly, if you are overweight, obesity, we say, is one of the most common factors in disturbing the process of ovulation. 

How Do You Determine The Ovulation Time? | Symptoms of Anovulation 

Now, how do we know that ovulation is about to happen? First, the body gives certain signs before ovulation. 

Body Temperature 

If we can monitor them clearly, then we can deduce whether the ovulation is about to occur or not. For this, we can measure the basal body temperature. Usually, there is a slight increase in body temperature during ovulation. But we need to measure it at the same time of the day and on an empty stomach. Because consumption of fruits or anything hot or cold or doing heavy exercise can alter the body temperature. 

Cervical Mucus 

The second thing is cervical mucus. During ovulation, the cervical mucus or vaginal discharge becomes sticky, transparent, and visualized just like a wire; if we note this type of discharge, we can predict the time of ovulation, or it will not happen. Apart from this, LH surge detection kits are available in the market. Using these kits, we can determine the time of ovulation, like when there is a rise in the luteinizing hormone inside the body. So these kits can detect the surge of the luteinizing hormone. But until and unless we confirm that ovulation has occurred. We can’t surely say that the ovulation process has happened or not.

Ultrasound Follicle Monitoring

There is another method known as ultrasound follicle monitoring, in which we continuously monitor the follicles to determine whether the ovulation has happened or not. The incidence of anovulation is about 30%. If I say 30% of body cycles are anovulatory and are the main reason for infertility. For their treatment, we have medicines that induce ovulation. 

Causes Of Ovulation Failure

Sometimes we see patients taking contraceptive pills for a longer duration or those who are taking 21or 24 days pills. So the condition of anovulation is also very common among them. Because the mechanism of these medicines is to suppress the ovulation cycle. 

If you have any of these problems, or if we could do regular exercise or reduce stress. And those who are overweight should reduce their weight. Those who perform heavy workouts should ease out their exercises and do them in ample time. 

And if you are unable to conceive, then you must consult your doctor. And must take medicines for anovulatory cycles.

Since we have more office jobs and desk jobs, we tilt our heads often to use mobile phones, and we use laptops more, so neck pain has become a very common problem. It is more commonly seen in the younger population, as old as 10-12 years to 50-60 years. 

Neck Pain Causes | Cervical Spondylosis Treatment

In Children 

There are many reasons associated with neck pain. In younger age groups or student life, it occurs because of bad posture. As we see kids bend to read their books in incorrect posture, and we are constantly using mobile phones, so mobile phones have become a common reason for neck pain. They keep their chins below while reading and even tilt their necks toward the telephone when holding it between the ear and the shoulder. So mainly in the younger population, the wrong posture causes neck pain. 

In young adults | Neck Pain Treatment

If we move to alight forwards, then in the office often people work on laptops, file work and bend their neck for a long duration, and after 30-35 years some changes start commencing in the body we see as cervical. But I would like to tell you one thing: cervical is not a disease; it is another name for the neck. So the cervical itself is not a diagnosis. Rather cervical spondylitis is a diagnosis where some age-related changes start commencing after 30-35 years. 

Like in some situations, we call knee joint problems arthritis, so when there is wear and tear of the spine joint, we call this spondylitis. So between 30-35 years, most of us have some changes like cervical spondylitis, and of course, after 50 years, there are degenerative changes inside the body. So these are some broad reasons for neck pain.

WHEN SHOULD WE OPT FOR TREATMENT?

If you are having only neck pain, then there is nothing to worry much but most of the time, what happens is that it commences with the neck and starts spreading towards the arms. 

Next, if we don’t treat it timely, cervical pain starts spreading to the arms and hands, which may lead to dizziness, numbness, and weakness in the hands. 

So, like the spine, the first questions are: 

  • Whether the pain is confined to the neck or has passed to the arm? 
  • Are you feeling dizzy?

If the pain spreads to the arms and the patient complains of dizziness, we take this seriously. In X-Ray, we can only visualize the bony structure, and if the patient has numbness and weakness, we do MRI. 

WHAT ARE THE TREATMENT FOR NECK PAIN?

We treat these conditions mainly by advising them on good sitting posture, sleeping posture, and exercise in the younger population. If the problem persists, pain is going towards the arms, or there is numbness and weakness in the hands, and if MRI shows the presence of a big disc. Like the back disc, there is a probability of a slipped disc. So in sporadic cases, we do surgery. 

Often we treat this condition with injection and physiotherapy. But most patients, especially those suffering from cervical spondylitis, do very well with proper sitting, sleeping posture, exercise, physiotherapy, and anti-inflammatory muscles relaxant. With these, we can heal this condition.

Doctors advice 

The doctor tells the  viewers some are sitting and sleeping postures to avoid neck pain. 

  • The first thing is to avoid looking low while working like I hold my mobile phone rather than watching while keeping it below; hold like this slightly straight and use it. Use it according to your eye level. Don’t bring your chin down; you can move your eyes below but not your chin. Not lifting the chin down is the most important thing to avoid neck pain. 
  • The second thing is that when you are lying down, many people don’t use the wrong pillow. You can use a slim pillow, but when you take a side, use a heavy pillow according to shoulder width and keep it below the shoulder to avoid neck hanging. 
  • Finally, avoid keeping your hand over your head while asleep. If you can do these three things and won’t keep your hand below if you have a heavy pillow around your shoulder, you need two or three pillows, a light for the head, and a heavy pillow. If you follow these tips, you can avoid neck pain to a much greater extent.

How is neck pain diagnosed? 

 For diagnosis, mostly when the patient complains about neck pain, we diagnose this condition by X-Ray most of the time. If it is cervical or cervical spondylitis, an X-Ray shows the required changes. And if the pain is spreading towards the arms and there is numbness and tingling in the hands, we advise MRI to determine the probability of nerve compression. If the patient is feeling dizzy, many patients say they feel dizzy when they do; I would like to add that dizziness can be another reason. 

So we need to rule them out, and if we suspect that it is occurring because of cervical pain, we do doppler, which is a type of ultrasound. It tells us about any vessel damage. So broadly, we do these three tests to diagnose cervical pain.

You must have commonly seen this disease among your relatives or friends. Since it is a chronic debilitating condition, patients usually get frustrated. As it requires lifelong treatment, the doctor would like to give some information about Rheumatoid arthritis using this platform.

WHAT IS RHEUMATOID ARTHRITIS OR GATHIYA?

It is a chronic inflammation of joints. It is different from osteoarthritis. Osteoarthritis commences with age; there is wear and tear on joints. The patient may have bowel, urine, or viral infection, resulting in arthritis in other conditions or some skin condition. 

There are some criteria for Rheumatoid arthritis. It is chronic; by chronic, the doctor means long standing; in some cases, it lasts life long. There is a chronic inflammation of the joint lining, due to which there are some symptoms that we consider a condition of Rheumatoid arthritis. 

There are some criteria for this 

  1. It affects multiple joints, mostly small joints like fingers, wrist, elbow, feet, and ankle. So this condition involves small and multiple joints.
  2. It can affect both bilateral or symmetrical; it’s not like if you have arthritis, on the one hand, it will not affect the other; it will simultaneously affect both sides. 
  3. There are specific blood markers like RA factor and anti-CCP. These are present in high amounts in Rheumatoid arthritis patients, and 
  4. It may lead to deformities in later stages, majorly in the hands and legs. 
  5. Other clinical symptoms like light or low-grade fever, malaise, weakness, and fatigue are very common and early symptoms of this condition. The patient feels malaise, uncomfortable feeling, weak, and fatigued. 
  6. Early morning stiffness is another very important criterion in Rheumatoid arthritis. Patients complain about stiffness in joints in the early morning. So, there is pain and stiffness, especially in the early morning. If any patients present the above symptoms, we encamp them in Rheumatoid arthritis.

HOW CAN WE MAKE A DIAGNOSIS OF RHEUMATOID ARTHRITIS? 

There are specific criteria for this condition’s clinical symptoms. Usually, we see signs in patients aged 30-60 years. However, there is a variety of Rheumatoid arthritis where it can commence early or late. But most patients are between the age of 30 to 50 years. Females are more prone to Rheumatoid arthritis, and they show symptoms of bilateral or symmetrical joints on both sides. 

Some clinical signs are early morning stiffness in the legs and hands, pain, swelling, inflammation, fatigue, and malaise. In addition, there are some clinical signs like nodule formation on the skin, and if we suspect it is Rheumatoid arthritis, and the symptoms have persisted for the last six weeks; then, we perform a blood markers examination.

Under lab investigation, we screen the RA factor test, but please remember this comes positive in only 80% of cases, and we perform another test which is anti-CCP. So it is very important to understand one thing about these tests.

Often patients come to us and say that their RA factor is negative. See RA factor is not positive in all the cases. There are 20-30% of cases that show negative RA test in blood but are a patient of Rheumatoid arthritis, which we call a seronegative variety of an  Rheumatoid arthritis. So next, we do further investigation to confirm our doubt by doing X-Ray of hands and feet. Erosion of joints seen in X-Ray helps in diagnosis. 

So it is the combination of clinical symptoms, lab tests, and Radiological Features.

WHAT IS RHEUMATOID ARTHRITIS? HOW DOES IT OCCUR? | Rheumatoid Arthritis Treatment & Symptoms

It is a genetic problem. Instead, it is a combination of genetic, hormonal, and environmental issues. We have specific markers or genes present in the blood which causes the autoimmune disorder. Our body has its immunity which fights against germs and infections. 

In the case of  Rheumatoid arthritis, the body generates immunity against its cells. Since it acts on its own cells, we call this condition an autoimmune disorder. In Rheumatoid arthritis, the body develops an excessive immune response against its cells. This immune response attacks the synovial lining of joints and causes inflammation. This response results in pain, stiffness, swelling in joints, nodule formation, and gradually, there is destruction in joints, and ultimately, a deformity stage commences in the hands and legs. 

One very common myth is associated with Rheumatoid arthritis. People assume that Rheumatoid arthritis only involves bones. Please remember that Rheumatoid arthritis is a disease of bones, muscles, connective tissue, and nerves, so much so that it affects the heart and lungs. Rheumatoid arthritis is like diabetes; it is a chronic condition that affects many organs. However, it is marked more commonly in joints and bones.

WHAT IS THE PROGNOSIS OF RHEUMATOID ARTHRITIS?

Rheumatoid arthritis is a chronic inflammation condition. Our purpose is to explain to the patient that we can control this condition, but we can’t cure it. It is just like diabetes; if their sugar is under control, the patient can lead an everyday life; the same goes with Rheumatoid arthritis. 

We can control Rheumatoid arthritis but can’t cure it. Because it is associated with genetic factors. Some risk factors are related to Rheumatoid arthritis like smoking, obesity, patients with a family history of Rheumatoid arthritis, and those who have long-standing problems. So these are a few risk factors, but primarily the prescribed medicines run lifelong. 

This disease does not remain the same, you will not have the same condition for a whole year. Like in the Raining season or winters, or changes in weather, the attack of pain and stiffness in joints comes suddenly. So we call this condition a relapse or flare-up. It can stay low grade for a few weeks or months, or a year, called remission. 

So the treatment of Rheumatoid arthritis runs between this flare-up and remission. This is why we callthis condition a long-standing chronic problem. Also, you that since it is a long-standing condition, people often get frustrated and search for other remedies, which most common is the consumption of desi dawai, which is nothing more than a high-dose steroid. So one should always be cautious. And definitely, one can try other treatments, but you must consult your physician first.

TREATMENT OF RHEUMATOID ARTHRITIS

We treat Rheumatoid arthritis in various categories: physical, rehabilitation, and occupational therapy. 

It commences with stiffness and deformities in joints. So physical therapy remains the mainstay of the treatment. Then comes the medical treatment. We have perfect drugs for Rheumatoid arthritis, but our purpose is not to relieve the pain. However, we wish to make the patient mobile and physically active and prevent the permanent deformities as much as possible or stop their progression. 

So the first line of treatment is pain killers or nonsteroidal anti-inflammatory drugs. We prescribe them during the initial stage of Rheumatoid arthritis, but we abstain from long-term use as regular painkillers consumption imposes side effects on the kidney and liver. So painkillers are the initial and first line of treatment in which we prescribe steroids. We prescribe steroids in low doses, and pulse-like meaning given in breaks are effective in this condition but avoid the consumption of a high amount of steroids. 

Although people take high-dose steroids in desi dawai, it may show its effect like magic, but it would lead to hollowness in bones, and you may suffer from multiple issues. So Steroid, when consumed only in proper doses, is effective for Rheumatoid arthritis.

Medications 

Next are disease-modifying drugs, or DMARDs are the primary medicine used for the sole purpose of preventing deformities in the legs and hands. Different medications, like methotrexate, vasospasm, and Hqtor, were used very commonly in COVID times. So these medicines are quite safe and effective, and we must use them in combination; with at least two drugs so that their side effects are minimal. 

Another thing we must understand today is that the doctor prescribes you the combination drug for Rheumatoid arthritis. However, it may stop showing its effect after a few months, so we need to be cautious and shift to the next level. So we must give drug holidays. We must give a break so that the body can recover from the side effects. And when we use the drugs in combination, these DMARDs are very effective. 

Then we have Biologics. These DMARDs and biologics bring change in the body’s immune response; they act like immunosuppressants so that we can control autoimmune disorders. Biologics are an expensive and stronger class of drugs. We use them widely. Then, of course, we have surgical management. 

Since rheumatoid affects mainly the main joint like the knee or hips, we surgically replace these joints with hip joint replacement or knee replacement. So there are specific indications of surgeries in the late stages of this, but by enlarging the combination of physical, occupational, and nutritional therapy, there are certain foods that we can aggravate so we tell the patient to avoid them. 

Surgical Treatment 

Apart from that, we have surgical and medical treatment whenever required. 

The doctor wants to explain to you about surgeries we perform for Rheumatoid arthritis. Suppose there are deformities in small joints like fingers. In that case, we should prevent them as their surgical correction is not very satisfactory. 

Surgical management comes when there is the involvement of big joints like the knee or hips. However, osteoarthritis also affects the knee. In this condition, knee damage relatively occurs at an early stage, so we need to do knee replacement at an early age group, somewhere between 40 and 50. 

If there is a complete deterioration of the joint and medicines are not giving relief, making you immobile, you should opt for knee replacement surgery. As knee replacement surgery is very successful, it modifies the quality of life. It is bloodless, painless surgery, and wherever your doctor examines that there is a complete deterioration of the knee with, please opt for knee replacement surgery. 

But yes, you must continue with your anti-rheumatoid drugs. This does not only affect bones; it affects muscles, nerves, pins, needles, and tissue, so the drug would help in combating the damage, and wherever needed, opt for surgical options.

Bottom Line 

So, in the end, I will advise my patients that they don’t get disheartened or frustrated because of Rheumatoid arthritis; you need to take long-term medicine for which you must consult your physician. Because when you are in a flare-up, there is an increase in the dose of medication. 

Otherwise, we maintain this with one or two drugs. Be cautious while consuming desi dawai as it has a high amount of steroids. Patients must continue their physical rehabilitation therapy, and a nutritional diet. Avoid smoking. Patients must take care of regular medicine and monitor their side effects simultaneously.

It is unique and challenging because unlike knee replacement is a very common surgery; everyone is aware of it. But hip replacement surgery is slightly rare as compared to knee replacement. We see it more commonly in younger age groups than knee replacement. There are a few challenges to the longevity and efficiency of this procedure. 

So today, the doctor discusses some developments pertaining to this topic. Our hip joint is like a ball-socket joint. This socket is mobile in the joint and is comparatively a strong joint. It is also like the knee and ankle weight-bearing joint and is a central axis of our body weight. Some reasons leave us with a choice of hip replacement. As I told you, hip replacement surgery is less common in comparison to knee replacement, and it occurs more in younger age groups. That’s why we face a few challenges during this surgery. 

WHAT REASON COMPELS US TO GO FOR HIP REPLACEMENT SURGERY?

One reason is injury or trauma; more commonly, we see the aged 40-50 have fractures near the hip joint. Furthermore, we can’t construct the fractures with the aid of implants or screws, or after a particular age, we opt for hip replacement. Another thing is if we have any old pelvis or acetabulum fracture that we could not reconstruct appropriately and is causing trouble now. So trauma is one of the main reasons. It can be an old injury or a fresh injury. 

Wear and tear of the hip joint 

The second is the wear and tear of the hip joint, like the knee joint. This condition is called osteoarthritis, and it is one of the indications for hip joint replacement. Finally, at a younger age, we see avascular necrosis. In this condition, the ball of the ball-socket joint becomes dead for some reason; there is a complete loss of blood supply to this joint. 

Lifestyle choices 

Many factors contribute to this condition, like alcohol, smoking, and intake of steroids, and in 80-90 percent, it occurs without any reason. In the younger age group, in their mid 40-50 start feeling pain in their hip joint, and after diagnosis by X-Ray, we infer that there is a loss of blood supply to that joint. 

So when the joint is dead, we suggest hip replacement surgery. There can be other reasons like infection and some diseases like arthritis, but broadly these are the most common reasons to opt for hip replacement surgery.

TYPES OF HIP REPLACEMENT SURGERY

Hip replacements are of two types: 

  • One type is a partial hip replacement or hemiarthroplasty, and 
  • Another type is a total hip replacement in which both ball and socket

As mentioned before it is ball and socket joint, so sometimes we only require to replace the ball, but it is scarce. Mostly we replace both the ball and socket joint. This type of replacement is called total hip replacement. This is how hip replacement in the younger population is done. 

Unlike previous replacements, which don’t stay for much time, only 10-15 years, the patient takes time to walk. But with the new advances, we make small incisions for surgery which is more commonly known as minimally invasive procedures. It is bloodless, painless surgery, and we make patients walk the next day after surgery. 

Different varieties | Total Hip Replacement Surgery

There are different varieties in this, like whether it is replaced by cement or not. In younger age groups, we prefer uncemented replacement because it is comparatively more straightforward if we need to change it after 15-20 years. 

These days we insert big-sized balls in ball-socket joints, and there is a change in the quality of metal that we use these days to increase the life of the hip by 25-30 years. 

New advancements 

With the new advances, this surgery has become very successful, a minimally invasive, bloodless, painless procedure. A patient can resume their regular activity within one month to 45 days. Hip replacement surgery is a very successful surgery if done timely. One more myth that I want to clear regarding hip replacement surgery is that earlier, people used to think that after surgery, they couldn’t sit on the floor or cross their legs. 

New techniques and implants 

As the doctor mentioned, they do this in the younger population, and with the newer techniques and implants, one can do all their activities like sitting on the floor cross leg and can do other similar work. You won’t face any problems in nearly 25-30 years. And God forbid if we need to change it again, it can change quickly.

How to control nose bleeding in children during summers?

What are the complications associated with airplane ears?

The severe complication one can have in an Airplane ear is bleeding. There can be bleeding inside the ear, or there may be rupturing of the drum, which might cause bleeding. And there can be ear blockage for a long duration. 

So these are a few complications that are rare but are possible. 

Who is at risk of developing airplane ears?

In this regard, I would like to say that those patients who have a previous history of such problems need to be careful. If they have faced any symptoms in the ears during flight or have acquired this one or two times. Then they are at risk of developing this again. 

And the second thing is that any patient who already has a cold, cough or viral, or nose blockage needs to travel by air is at risk. That is why these people need to take extra precautions. They need to keep swallowing, keep doing Valsalva flying, keep on licking something so that they keep swallowing their saliva, and possibly take painkillers.

  

How is airplane ear diagnosed?

Diagnosis is not difficult as you have already experienced it before this. There will be some findings of the middle ear or ear drum; the doctor might detect fluid accumulation in the middle ear. You might have inflammation in the nose or you have developed sinusitis or URI (Upper Respiratory Infection). 

We can visualize all these clinical conditions by endoscopy. So clinical examinations are necessary to diagnose airplane ear. In addition, we do some tests for audiometry to check whether there is negative pressure or fluid in the ear or the drum is moving. Then we can perform all these tests for diagnosis.

How common is airplane ear?

The airplane is not that significantly common because in our body, nose, or ear we have appropriate mechanisms that can easily handle changes in pressure. For example, the tube that opens in the nose from the ear is efficient and does not get blocked easily. But those who are already having an infection or are allergic tend to acquire frequent infections; then, it is common among these patients. 

So they must be little aware of this condition. But, overall, in the general population, if 200 passengers are traveling in airplanes, one or two may have this problem. So it’s neither common nor uncommon.

Mostly we see it in younger people with a history of falling; it could be an accident or any fall or very common in sports injury; often, sportsmen say they had shoulder dislocation. The patient comes to go to the doctor with intense pain and a frozen shoulder (inability to move). 

Usually, they come go to the doctor in an emergency by supporting their arm with another hand, keeping it a little farther from the body and slightly tilted upwards.

TYPES OF SHOULDER DISLOCATION | Shoulder Dislocation Treatment Surgery

Anterior dislocation

There are two most common types of shoulder dislocation: anterior dislocation, seen in 90% of cases. Anterior dislocation means our shoulder joint is like a ball-socket joint, so when this ball comes towards the front, the doctors call it anterior dislocation. 

Posterior dislocation

And if this ball moves backward, leaving the socket behind, which we call this posterior dislocation. This ball may sometimes move towards the bottom (inferior), but we often see anterior dislocation followed by the posterior and inferior. As Dr. Manpal mentioned, it usually occurs due to an injury which could be due to an accident or fall or sports injury, and the patient has severe pain. 

Anterior dislocation is a more common type of dislocation after an injury or fall, whereas posterior dislocation is rare. It usually occurs after convulsions, like if the patient has an epileptic fit or ECT therapy, their arm moves inside for any reason. 

So the ball rolls towards the backside, which we call posterior dislocation. Clinically patient comes to us with severe pain, unable to lift their arm, and the shoulder becomes slightly flat as the ball comes towards the front. If the patient is very lean, we can easily visualize the ball coming out in the front.

Diagnosis 

Usually, patients present their arms like this. It is a severe condition. So in an emergency, when we see the patient after an injury or fall, the first thing we do is manage the pain by administering a pain killer, and then we go for an X-Ray scan. There is a clinical test in which we can diagnose anterior dislocation quickly since they cannot touch their other shoulder.

The patient cannot move their twisted hand, so clinically, when we see a condition where the patient cannot touch the opposite shoulder, we suspect it could be shoulder dislocation. In X-Ray, we can visualize the dislocation of the ball from its socket. And then, we plan for treatment.

TREATMENT OF ANTERIOR DISLOCATION

In 90 % of the cases, we perform a close reduction. By that, I mean we fix it with particular maneuvers, but here I would like to request you to please visit your orthopedic surgeon because I often hear patients saying they had a shoulder dislocation, went to quack, and healed it.

Sometimes there is no dislocation. Instead, it is soft tissue injury, but quack people interpret soft tissue injury as shoulder dislocation and treat it. So please beware of all this. I want to caution you all about this. Whether you have an injury, it could be a rotator cuff or any damage, they visit the quack, consider everything as dislocation, and treat it accordingly.

But this is very untrue. A shoulder dislocation is an acute event; it’s not like it occurs without reason. There is always a history of injury or fall, or trauma. We can diagnose shoulder dislocation by X-ray. Once we diagnose it, we make a closed reduction. We perform this procedure under anesthesia, and there are some maneuvers, and we stretch retraction or press our foot over the armpit and pull the arm. you can reduce it with a click. It’s a 15-20 minutes procedure that the doctor performs under anesthesia.  

Then we tie the patient’s arm with the chest for 3-4 weeks. So this treatment works in 90-95% of cases. First, we immobilize the injured arm. In scarce situations, it doesn’t reduce; then we plan a surgery. We do open reduction surgery in which we make an incision and fix the ball in a socket. If the patient comes to us at an early stage in the first 4-5 days or roughly 2-3 days, we can heal it by closed reduction. Only if the patient comes to us very late or the patient is very muscular or obese then the ball doesn’t fix in the socket on its own. 

COMPLICATION OF SHOULDER DISLOCATION

There can be some fractures associated with shoulder dislocation. As I mentioned earlier, it’s a ball and socket joint, and when there is dislocation of the ball, there is a tear in nearby ligaments, muscles, soft tissues, and capsules. Only then the ball would dislocate. Otherwise, it is a solid joint wrapped firmly inside muscles and ligaments. So there can be an injury to muscles and ligaments. 

Along with this, there can be fractures in some portions of the ball. There might be fractures in the cup. But most of the time, these fragrant fractures heal on their own when we fix the ball into the socket. Again under these circumstances, we don’t require surgery after reduction.

When we do an X-Ray, we see the ball inside the socket. And the part of the fracture has come back to its original position. So most of the time, surgery is an option. 

But we operate in some cases where the fracture doesn’t sit in its original place. Or the dislocation does not reduce.

 WHAT IS THE COMPLICATION OF ANTI-DISLOCATION? | Shoulder Dislocation Treatment Surgery

  1. When there is unreduced dislocation for whatever reason, we are unable to reduce the dislocation. 
  2. When there are fractures associated with a dislocation. Under these circumstances, we need to operate it to fix it. 
  3. Recurrent dislocation, which we often see in our OPD practices. Often we hear patients saying that they had shoulder dislocation 2-3 years back. And it gets dislocated again after one year while doing some particular activity. 

Further, they complain of repeated dislocation about 3-4 times. And if they take their hand towards the backside like combing hair or picking something from above, there is dislocation. The shoulder gradually starts to dislocate with minimal force. So they always feel that either there is a dislocation or they have the feeling that the shoulder will fall. 

So the doctors call this condition a recurrent dislocation. It is a prevalent complication. And it often occurs when we don’t immobilize it adequately with a splint during the first incidence of shoulder dislocation. The primary purpose to use splint is to heal the soft tissues adjacent to the joint. If we don’t immobilize it for enough time, then shoulder dislocation turns into recurrent dislocation. 

And please remember that once the doctor labels your shoulder injury as a recurrent dislocation. You will require the need of surgery. We have a perfect surgical option for this. These days we repair it with arthroscopy with the help of an endoscope. It gives good results, especially in athletes who require an excellent functioning shoulder. 

So recurrent dislocation is widespread, and we can treat them with arthroscopic repair, which yields good results. Please don’t ignore these injuries whether you need closed reduction or operative.

Consult your orthopedics surgeon when you have an injury and must go for a follow-up. One must follow Post-operative rehabilitation and physiotherapy very regularly and carefully.

What Is The COPD flare-up Or exacerbations? 

It means that COPD is becoming a dangerous situation, and you need to get hospitalized.

How will we know that we have COPD flare-ups or exacerbations?

Suppose there is an increase in the frequency of coughing, which was not present under normal conditions. 

In that case, you have a: 

  • fever
  • trouble breathing
  • severe headache
  • swelling in the legs 
  • suffering from mental confusion
  • drowsiness. 

If you observe any of these symptoms and you cannot perform your regular daily routine that you were able to do previously. If you observe any of these symptoms, then this means that you are in a flare-up of COPD or exacerbation. 

The doctor have seen bluish coloration on the tongue, hands, or nails in some cases because of sudden oxygen depletion. So this is an immediate indication of hospitalization. It’s important to understand these flare-ups since our diseases start getting worse as soon as they reach the flare-up stage. 

There will be a drop-in level of oxygen drop in oxygen saturation, so patients require adequate treatment under these circumstances. The doctors often take reliever medication or inhalers more frequently due to shortness of breath. Taking frequent inhalers is also one indication of COPD flare-up. So it is essential to take adequate treatment from the hospital.

What Are The Treatment Modalities Available For COPD Flare-Ups Or Exacerbations?

The main goal of all the COPD-related treatment modalities is to reduce the patients’ symptoms and give them a quality of life which is very important for them. And the doctors need to minimize these exacerbations or flare-ups of COPD. 

So to initiate the treatment, they prescribe different types of medicines, which they call bronchodilators or anti-inflammatory steroids. 

Under bronchodilators, they have other inhalers in which they either use long-acting beta-agonist or long-acting anti-muscarinic agents, commonly known as LAMAs. They usually are also given to the patients when they are at home. Otherwise, if the patient has severe flare-up or exacerbation, they use steroids. 

However, the use of steroids depends upon the indication of the patient, the type of pain, and the problem. Apart from this, a Pulmonary rehabilitation program is essential for these patients.  

What do you mean by pulmonary rehabilitation programme? 

I mean, it’s essential to have an exercise routine for these patients through this program. These patients get stuck in a vicious cycle of dyspnea when they have shortness of breath when they sit. And they think they can’t do anything, but because of this, they suffer more from shortness of breath which would add to their worries. 

And in the end, it would lead to depression and anxiety. So in this, the doctos ask them to do an exercise training program in which they teach different types of exercise to the patients. In this regime, breathing exercise is very important, which could be pursed-lip breathing or belly breathing. 

Breathing exercise helps in dyspnea because this exercise brings back the action of diaphragm muscles and helps the patients cope with shortness of breath. 

Apart from all, quitting smoking is an essential component of COPD treatment. No medicine will show its effect if you continue smoking. So you must start your program of quitting smoking under a doctor’s supervision. 

These days the doctors have different things like nicotine gums and other things available. But most importantly, they should make up our minds to quit smoking; otherwise, no medicine could show its effect. Another thing we have is the role of immunization in COPD.

What is the role of immunization in COPD? | COPD Exacerbation | Flare Ups

Immunization plays a vital role in preventing severe hospitalization and infection. Like Influenza vaccination which must be given annually to the patients, or pneumococcal vaccination, which is available in two forms according to the age: PCV13 and the other is PCV23. The immunization prevents severe pneumococcal infection or pneumonia and reduces the need for hospitalization. 

In addition, these days, COVID-19 vaccination is also very important for COPD patients to prevent further worsening of the situation. These patients need to administer Diptheria Zoster vaccination to prevent shingles. 

It’s essential to identify all these symptoms and follow the routine. There should be a plan of treatment made by a specialist doctor so that you can manage your exercise program and immunization schedule. 

And further, you can prevent COPD from getting worse because it is a preventable and treatable disease.

DIFFERENT STAGES OF ENDOMETRIAL CANCER:

Endometrial cancer is divided into four stages:

Stage one (Endometrial Cancer Treatment)

In the first stage, the tumor is restricted to the innermost or muscle layer of the uterus only. It has not spread into nearby locations. 

Stage two 

The second stage of endometrial cancer has reached the mouth of the uterus i.e. the cervix or cervical stroma. But it has not gone outside the uterus. 

Stage three 

In the third stage, cancer extends to the outside of the uterus but remains confined to the pelvis. By pelvis, I mean it is limited to either ovary, fallopian tubes or round ligament, and if it grows further, it will move to lymph nodes. Lymph nodes are small vessels on arteries and veins, either pelvic or para-aortic lymph nodes. This is our third stage. 

Stage four

Finally, cancer spreads outside the pelvis in the fourth stage of cancer. It can be in the lungs or bone or has metastasized into any other body organs, which the doctors call distant metastasis. In this stage, it starts spreading into adjacent organs of the body. 

Now the doctors will talk about its treatment.

TREATMENT OF ENDOMETRIAL CANCER

Surgery 

The doctors can diagnose this uterine cancer early in most cases because most patients present heavy bleeding. If the doctors can analyze it early, the essential part of the treatment is surgery. In surgery, the doctors remove the uterus, ovaries, tubes, and lymph nodes present in arteries and veins. And wherever the doctors see cancer has moved in the abdomen, they remove it. So surgery is the most important aspect of the treatment of endometrial cancer. The specialists should consult a cancer expert or expert gynecologist for this type of surgery as they require staging surgery. 

Radiotherapy 

So this surgery is critical. Other treatment modalities are radiotherapy, in which the expert expose a laser to stop the cancer cell division. Chemotherapy is another treatment option for endometrial cancer.

Medication 

The specialists use a specific type of medicine to stop cell multiplication. With the latest advance, they have hormone therapy. Since the imbalance of hormones has led to this situation to combat, they use certain hormones to overcome the effect of others causing imbalance. 

Targeted therapy 

Apart from this, the doctors give targeted therapy. As the name suggests, they target only the cancer cell, stop their multiplication, and inhibit their growth. Largely the treatment of cancer depends upon the stage of cancer. 

Often patients require the need of further treatment after surgery. But all these things depend upon the stage of cancer, type of surgery, and other modalities of treatment that will benefit them and increase the survival rate. You need to discuss this with your doctor in detail and make the best decision about the most suited treatment plan.

What are Airplane Ears? 

Pain in the ear while traveling on an airplane is a symptom that is not uncommon. Many people who travel by air feel intense pain in the ear. Especially they feel pain and ear blockage while the airplane is ascending or descending. So this condition is called Airplane Ear. 

So an airplane ear is called barotrauma of the ear or we call it aerotitis media. The airplane ear occurs mainly due to the change in pressure. There is an increase in negative pressure in the middle ear, which leads to acute pain.

What causes Airplane Ears? 

The leading cause among these patients is the one who has frequent upper respiratory tract infections like colds or viral. Therefore, they are more likely to feel this pain during air travel. And this pain is usually accompanied by ear blockage. The Eustachian tube connects the ear to the nose. In patients with a viral infection, upper airway infections like coughing, cold, or allergy, cause inflammation in the connecting tube as well. 

Due to this, whenever there is a change in pressure or an increase in the negative pressure, there is a blockage in the connecting tube. This blockage creates a vacuum inside the ear, which causes a blockage, and one feels shooting pain. Shooting pain is also known as vacuum pain. It occurs due to inflammation of either in-ear tissue of the middle ear; due to negative pressure, there is congestion in the vessel, and one feels pain due to the negative pressure.

How to avoid ear pain during flight?

Valsalva Manoeuvre | How to Avoid Ear Pain During Flight

To ease this pain in an airplane, we advise our patients to do the Valsalva Manoeuvre during flight so that they don’t have this problem. So what do we do about this? We hold the nose and exhale and try to open our ears. If the ear keeps opening during flight during exhalation by the nose, then there is significantly less chance of an airplane ear. If you cannot open your ears by following Valsalva, the air is not going inside; then there is the probability of having ear pain or ear block.

Keep swallowing something 

The second important prevention is to swallow something constantly. And you must have observed on airplanes that air hostesses announce and they often give you toffee or water. The purpose behind this is that the passengers have to keep swallowings something. During swallow motion, the Eustachian tube frequently keeps opening, allowing air to enter, so there is less chance of an airplane ear. 

Carry Painkillers 

Third thing if you are infected with a viral or cold, or cough, so under this situation, and you are taking a flight, then you must have decongestant or nose drops and medicine. It would help if you consumed them before taking flight so that there is minimum inflammation and less chance of forming negative ear pressure.

All these patients must carry painkillers with them so that whenever they feel pain, they may take painkillers, especially before descending the aircraft. Because we have seen that often people have more problems during the ascent(take off) and descent(landing) of the aircraft. Patients who have a cold, especially those who have a previous history of airplane ear, should have painkillers or anti-inflammatory before taking a flight to avoid severe intense pain. There can be blockage of the ear, but you won’t have severe pain. 

Consult a doctor 

After you land or reach home, the ear pain heals in 1-2 days because you are already taking medicines for the cold. But suppose if the ear block doesn’t heal. Like in some cases it persists for a longer duration, then they must consult their doctor. Because most likely, there is a fluid accumulation in the middle ear due to negative pressure, which might get infected or remain sterile, leading to continuous blockage in the ear. 

And if it doesn’t open even after the Valsalva exercise. So in these cases, there is a requirement for medical intervention; we prescribe patients with antibiotics and decongestants, and in a few cases, the doctors give steroids and nasal decongestants. So these things can help in controlling middle ear infections. In a few rare cases, the doctors need to perform ear intervention in which we insert one tube inside the ear to restore ventilation and airplane ear heals.

WHAT IS ENDOMETRIAL CANCER? 

Endometrial cancer, as the name suggests cancer of the endometrium. The endometrium is the innermost layer of the uterus, the middle layer of the uterus is the myometrium, and the outermost layer is the serous layer. 

For any reason, if there is an uncontrolled growth of cells in the innermost layer of the uterus, then we say it is endometrial or uterine cancer. It is prevalent cancer in females. 

Earlier its prevalence was more in the western world, but now a days, its incidence has drastically increased in developing countries like India. Due to lifestyle modification, lack of physical activity, and excessive hormonal imbalance, the incidence of endometrial cancer is more in Indian females. 

First, we will talk about the risk factors associated with endometrial cancer.

WHICH PATIENT COMES UNDER THE RISK CATEGORY?

Among high-risk patients are those who have:

  • a high concentration of estrogen in their body for any reason
  • are obese
  • a Body Mass Index of more than 25 
  • are suffering from diabetes or hypertension
  • never been pregnant
  • any baby 

are maximum at risk of endometrial cancer. 

Apart from this, if you have a history of breast cancer or doctors have prescribed you medicines that have a direct effect on the uterus lining may put you at risk of uterine cancer. Patients with lynch cancer, in which there is a combination of colon and endometrial cancer, are also at a very high risk of endometrial cancer. 

Often patients ask in OPD how they would know or what test they should undergo. What kind of signs and symptoms do we observe?

WHAT ARE ITS SIGNS AND SYMPTOMS?

Endometrial Cancer Symptoms, Endometrial cancer, I would say, is one cancer in females that gives signs and symptoms of its onset in the form of uterine bleeding. We divide the menstrual cycle into three groups; In one group, we females are premenopausal with regular periods. Then we have the second group of perimenopause inclined towards the end of their periods, and the other group is at the menopause stage. The bleeding presentation in all three groups is different. 

Premenopause (Endometrial Cancer Symptoms)

In the premenopausal group with regular periods, if you have heavy bleeding, which usually doesn’t occur. Or may start bleeding again after periods stop, which we call inter-menstrual bleeding, these two critical presentations or signs that one must consult a doctor. 

Perimenopause 

Those in the perimenopausal stage are at the end of their periods; if suddenly there is a reduced frequency of periods according to their age, or they are having delayed periods or have minor bleeding may also be at risk. But if you have heavy bleeding or periods start coming fast, you must consult our doctor. 

Abnormal bleeding

There may be other reasons associated with heavy bleeding or early periods. We can’t always relate them to endometrial cancer, there might be some other reason related to it, but we must evaluate ourselves with the doctor. Many times we diagnose cancer in simple things. On average, ten may get diagnosed with endometrial cancer among a hundred women with abnormal bleeding. Under these circumstances, we must consult our doctor. If they advise biopsy, we should go for a biopsy. 

Post menopause group 

Next, we are coming to the postmenopausal group, where periods have stopped for almost one year. If these patients have bleeding or spotting, we must consult our doctor immediately. We must examine ourselves properly, and if they advise ultrasound, we should go for an ultrasound. And suppose we should opt for biopsy after considering ultrasound. In that case, the thickness of the endometrial lining, or some other reason if the doctor has advised you for biopsy, then we must go for a biopsy test. This was all about the bleeding patterns. 

Signs and symptoms 

Many patients who have endometrial cancer may suffer from stomach aches, which we call pelvic pain, or they may have a bloating session. Apart from this, they feel the change in their urinary habits like the frequent urge to urinate, or there may be changes in bowel habits like they may suffer from diarrhea or constipation. Apart from that, they have general signs like losing appetite and weight loss, which are some common signs of endometrial cancer.

What do you mean by COPD? 

It’s a Chronic Obstructive Pulmonary Disease that primarily affects the lungs. It is very commonly present, and it is both preventable and treatable. 

WHAT ARE THE PROBABLE CAUSES? 

Smoking is the leading causative agent in 80-90%of cases. It could be in tobacco smoke, cigarette, bidi, or hookah. So COPD is majorly associated with all these things. A person with passive smoking who lives near an active smoker is also at risk of COPD. Apart from smoking, outdoor air pollution, which includes smoke from factories, gases, and gases from on-road vehicles, is potentially hazardous that may lead to COPD. 

Then there are occupational exposures, other toxic chemicals, paint, explosives, and people working in such factories may acquire COPD. In a few cases, there can be genetic reasons like they may have a alpha1 antitrypsin deficiency, or in the early childhood period, the kid was prone to frequent throat infections then, which may lead to COPD in later stages.

HOW DO WE KNOW WE HAVE COPD? WHAT ARE ITS SYMPTOMS?

Frequent coughing with phlegm, clearing your throat all the time, shortness of breath while walking, tightness or pain in the chest; are a few symptoms; if you see in yourself or you are a habitual smoker, then you must keep all these symptoms in mind and you must visit your specialist for its treatment.

HOW CAN WE DIAGNOSE COPD?

The doctors perform a lung function test whenever any patient presents with the following symptoms. The lung function test is commonly called spirometry or pulmonary function test. The doctors measured their lung volumes, like how much air they can exhale in one second or six seconds. 

Accordingly, we diagnose it. And if it is less than thirty percent, The doctors analyze the patient’s condition as COPD. Apart from spirometry, we perform ECG and X-Ray as it is associated with many other conditions. An X-Ray may show some changes from which we could infer the stage of COPD. 

WHAT ARE THE DIFFERENT STAGES OF COPD?

There are four different stages of COPD, and based on lung function tests, which is categorized COPD into mild, moderate, severe, and very severe. In addition, when we talk about symptoms,  there are specific scales of symptoms. For example, according to the GOLD Science committee, we categorize the symptoms in four quadrants: A, B, C, and D.  

In the A category, patients have significantly fewer symptoms. There is no flare-up in category A which we will talk about later in this video, and category D happens to be the most dangerous with severe symptoms, and their condition is deteriorating. 

So, according to this scale, we identify patient symptoms and plan the treatment accordingly. 

So the doctors  advise any person who is active smoking or passively inhaling smoke and you have these symptoms, then you must consult your pulmonologist so that we can diagnose the disease at an early stage and can start treatment accordingly. In this way, you won’t be suffering In the later stages of life.

What causes nose bleed in children? 

Due to nose bleeding in children, often parents have anxiety because they are scared to see the blood coming out of their children’s noses. And why it is bleeding. Dryness is the most important cause of nose bleeding in children in this weather. Because of dryness, children often prick their noses, get irritated, and keep bugging their noses. 

Due to dryness, the tissue inside the nose becomes brittle and soft, ultimately leading to rupturing of blood vessels. It is impossible to stop children from doing this. Often, at bedtimes, they rub the inner lining of the nose unknowingly in their sleep as there is dryness in the nose. So constant rubbing leads to injury.

Many patients bleed from their noses without damage, but dryness is causative. So this nose bleeding occurs in children.The severity of nose build can vary from mild to very severe. And significant bleeding can lead to life-threatening conditions because children don’t have a blood reservoir. Blood loss may lead to situations like severe anemia in children. If these attacks are frequent, it is very important to treat them as excessive bleeding can lead to problems. 

What causes nosebleeds in adults? 

Similar pattern of nose bleeding is seen in adults. Nose bleeding in adults in the summer season is not very significant in most of the population. The causative reason behind this is dryness. But nose bleeding is mild and easily manageable in adults. We see severe nose bleeding in adults for maximum time in winters. In winter, we see more old-age patients in our OPD practice. 

In the winter there is shrinkage of vessels in old age as they get brittle with advancing age. Blood pressure, medicines, and atherosclerosis cause brittleness in vessels, or a malformation in the nose, leading to rupturing of these vessels. Even there is rupturing of vessels by loud sneezing or by taking deep breaths or blowing or by poking a finger, which might lead to significant bleeding. This excessive bleeding may be life-threatening if it occurs frequently. 

Reasons for nosebleed | Nosebleed In Summer 

Nose bleeding is categorised into two categories: one is summer bleeding caused by dryness, and the second category of nose bleeding, which occurs because of old age. There is either shrinkage of blood vessels or any malformation that results in excessive nose bleeding in old age. 

HOW CAN WE PREVENT NOSEBLEED? 

In children | Nosebleed In Summer 

In children with frequent nose bleeding, we instruct them to learn to pinch their nose, especially in conditions where immediate treatment is not possible, like if the child has nose bleeding at school or playing outside. So under this situation:

  • We must make them learn about pinching their nose. Bleeding will stop in 2-4 minutes when they will pinch their nose by keeping their nose between finger and thumb, and then you can go for their treatment or first-aid. 
  • Second thing we should cut children’s nails to avoid injury to the nose. Because they reflexively rub their nose or prick it, it’s difficult to control in children; since they don’t remember it that much, we should cut their nails to avoid injury to the nose. 
  • Children with a high bleeding tendency should use moisturizers, saline spray, or regular drops to avoid dryness. In addition, 
  • We can lubricate their nose with vaseline or cold cream, almond oil, olive oil or Neosporin, or paraffin-based ointment, which helps significantly. 

If the bleeding is unmanageable and there are frequent episodes of excessive bleeding, then you visit your ENT expert. The doctor may first rule out all the allergy-related problems, and they will find the vessels and seal them by some instrumentation. 

With the help of instruments, we find that burst vessel in the nose, and we burn or seal them. This procedure is called Cauterization. This Cauterization can be chemical or electrical. So this practice is essential for children.

IN ADULTS

In adults, nose bleeding is frequent in winters or can occur due to uncontrolled blood pressure, which can happen in any season. In these patients either we need to do nasal packing if we cannot find the reason for the bleeding. 

We search for the reason for bleeding by endoscopy, and if it’s accessible, the doctor immediately either seals or cauterises it. They normally cauterize with the help of instruments or endoscopes. 

So this is a long-term solution. Nasal packing is a short-term solution; if the doctor doesn’t have the facility at their clinic, it’s best to pack the nose with medical sponges.

And most patients heal after 2-3 days; otherwise, the best solution for nose bleeding is endoscopy and cauterization, in which we find the bleeding vessel and seal it.

Our experts talks about the most missed entity, a Muscle tear. People oftent miss it because often, thay ignore this condition; and the ankle ligament injury is not correctly diagnosed. And more importantly, we either overlook its treatment or treat them wrongly, and a few times, there is a delay in treatment. So it’s a nagging injury that may seem harmless, but it is not very pleasant. And it may last for years.

Almost every day, patient come in with a twisted ankle. They say they twisted their ankle six months ago, but now they are having muscle tears, swelling, and pain. The ankle has become unstable, which might irritate the patient for years. That’s why it is a missed entity. So we call this condition an ankle sprain. 

WHAT IS AN ANKLE SPRAIN? | Sprained Ankle Treatment 

The Ankle sprain is a ligament injury. Ligaments are soft tissue, but these are our elastic and intricate structures that connect our bone to another bone. We call them ligaments. And they hold significance in the legs because they connect our leg bone to the ankle. As you can see in this model, here is our foot, and this arrow is our leg. These ligaments connect our legs to our ankle and hold great importance; since they provide movement and stability to the ankle. 

A tear in a ligament happens to be very painful for the patients. Most patients come to us and say that they twist their ankles while walking on uneven surfaces or women wearing heels, and this condition is very common in sports. 

Complete or partial ligament tear 

As mentioned earlier, ankle sprains occur when you miss your step while running, jumping, and skipping. Also, we twist our ankles while climbing the stairs upwards or wearing heels. So the ankle twists slightly inside. So all these things lead to slight movement of the ankle towards the inner side. And when they twist inside, there is a tear in the ligaments. 

It can be a partial or complete tear or a simple sprain. So as mentioned, these are the elastic structures that contract, but if we stretch beyond the limit, they will break. It will allow movements like a rubber band, but it will break if you try to push beyond its limit. Similarly, a tear in ligaments is commonly called an ankle sprain.

HOW CAN WE DIAGNOSE IT?

Often patients with twisted ankles complain that they have sore ankles, difficulty in putting pressure, and there is swelling outside the ankle. Naturally, they are in much pain. 

Unfortunately, we can’t visualize these ankle sprains or ligament injuries in X-Rays. Only bones are visible in X-Rays, due to which these injuries are either ignored or overlooked. Often patients get X-Rays from local areas, and then they say that the X-Ray doesn’t show any bone injury. They think it is not a big deal if we do not have any bone injury. 

Please note that the ankle or knee is our weight-bearing joint, and this ankle is a tiny joint that has to bear all the body’s weight throughout the day. The ligament delicately balances the ankle joint. If there is an injury in the weight-bearing joints, it will hurt more than the fracture. Fractures may heal, but the damage to the ligaments is bothersome. After ligament injury, please don’t sit at home.

The doctor usually applies crepe bandages for two days, thinking it will be fine and the patient can go back to work. Untreated ankle sprain might irritate you for months to years. What you do in the first three weeks matters the most. We mostly don’t need an X-Ray, but we advise the patient for an X-Ray to rule out the possibility of a fracture. So this is a clinical diagnosis where we examine the patient’s activity, ankle movement, and whether there is a complete tear or a partial tear. Ligament injury is a clinical assessment, and an X-ray can’t diagnose this condition. After diagnosing an ankle sprain, the second step is its treatment. 

ANKLE SPRAIN TREATMENT | Sprained Ankle Treatment 

The treatment depends on the extent of the injury and whether there is a partial or a complete tear. How much is the pain and swelling at the same time? Remember that you must follow RICE treatment wherever there is a muscle tear, or you have some injury. If we follow RICE treatment for initial 24-48 hours, all the symptoms will reduce. R stands for Rest, I for Icing, C for Compression, and E for Elevation. 

After any muscle injury or similar injury, you may start RICE treatment by yourself within 24 hours of injury before coming to the doctor. RICE treatment will reduce the course of injury and symptoms. It is necessary to rest in this, and if your ankle is hurt, then the only way to relax is to either sit on the bed or rest on the chair so that it doesn’t bear the body’s weight. 

As mentioned before, I stand for Icing; it is the most critical factor to reduce your pain and inflammation. No amount of pain reliever can help you ease the pain compared to icing. One should do icing for at least 48-72 hours. The next question is how long we should do the icing.

HOW LONG SHOULD WE DO ICING?

One should never apply direct icing to the injury. You can either use ice wrapped in cloth or ice packs readily available in the market. Direct ice application may cause skin burn, so you must use ice packs or ice wrap in cloth for 10 to 15 minutes, and repeat this four to five times. You can do it six to seven times on the first day of injury. 

Otherwise, repeat for 4 to 5 times for 10 to 15 minutes; you will get much relief. As mentioned, R stands for rest, I stands for icing, and now C stands for compression; otherwise, we can also use a splint. 

Usually, the patient may apply a crepe bandage after injury before going to the Doctor. And they can use braces too. E means Elevation. The more we keep the ankle elevated, it will reduce the swelling. The idea of ​​​​elevation is to keep it above the level of the heart. You can keep two pillows below the ankle. So if you do all these things for 48 hours, it can shorten the symptoms upto a great level. 

WHEN SHOULD WE SEEK THE DOCTOR? | Sprained Ankle Treatment 

Ideally, you shouldn’t ignore any ankle sprain. As the doctor said earlier, this is a missed injury. You must always consult a doctor. Initial treatment will remain the same, i.e., RICE which we can do at our respective homes. In today’s era, we treat ankle sprains just like fractures. We advise the patient to take a plaster because we need to immobilize it. 

Like instructed, I in RICE stands for icing, but there is immobilization also. If you broke something, then it is necessary to keep it in one place; if the same thing keeps on moving, if you keep putting on weight, then an injury that would cure in three to 4 weeks might take 3 to 4 months to fix and  even longer. Either you can use a brace or ankle splint.

In most cases, the doctor does recommend plaster for 3-4 weeks. And these days, there are fiber cast plaster which allows a little weight so we can even walk easily with the help of a walker. So if you do all these things, you can treat ankle sprain to a much greater extent. 

HOW LONG IS THE ANKLE SPRAIN RECOVERY TIME?

It depends on the muscle tear; is it a partial or complete tear. According to the doctor, we can’t diagnose ligament injury by X-ray. However, you can interpret this by MRI, but there is no need to do MRI in every case. At first, the doctor does plaster, but if it is not working, the pain has not receded in the first 3 to 4 weeks, then he feels that there is a pervasive injury in which the ankle has become unstable. 

Then the doctor does MRI, but the MRI is not required as much. These ligament injuries heal in a few weeks if they are partial or minor injuries, but it may take more than three months or even years to recover. For three months, you have to take care of yourself; even if the plaster is removed, you need to take care of yourself, walk properly, and exercise. 

WHAT IS THE FUTURE TREATMENT? | Sprained Ankle Treatment 

When we put plaster for 3 to 4 weeks, it is not that we can immediately put weight on it; we advise them to do gradual strengthening exercises. Just like I gave an example of a rubber band. While stretching, we break the rubber band; then it will recover in an extended position; if it does not get the rest, then our rubber band will remain loose; so the same thing happens with our ligament. 

If the ligament remains loose, they either will not join again or might join in an extended position that will be painful. So this type of ligament will lead to two things: pain or swelling, or the ankle will remain unstable and may twist every 2-3 weeks. So after we remove the plaster, you should do some ankle strengthening exercises to avoid this recurrent ankle twisting.

WHEN DO WE NEED THE SURGERY?

Only 2% of the cases require surgery for an ankle sprain. However, where there is an extensive ligament tear or ankle stability, even after plaster and exercise, the pain is not receding, then we recommend surgery in 5% of cases. 

But in most cases, it can be healed by the RICE treatment followed by plaster and exercise. So please don’t ignore your ankle sprain; we can cure it very well if it is treated well in time, and anytime you feel that there is a muscle tear, you must seek the  Doctor who will advise you correctly. First, we need to understand the meaning of the term Bartholin Cyst. Any water-filled clot is termed a cyst in medical language. 

WHAT IS BARTHOLIN CYST (बार्थोलिन सिस्ट)?

Bartholins are two pea-sized glands present at the vaginal opening. Their function is to maintain lubrication in the vagina. These glands release their secretion by their duct. 

Bartholin cyst treatment now, if there is a blockage in this duct, these secretions will not find their way out and will keep accumulating in the gland. Eventually, this would result in the formation of a cyst. This type of cyst is sized approximately from 1 to 10cm which means it may take the shape of a golf ball or lemon from a pea. 

WHAT ARE THE COMPLICATION ASSOCIATED WITH BARTHOLIN CYST?

Often patients come to the emergency with the indication of intense pain. The most important symptom of this cyst is severe pain in the cyst. Apart from pain patient may feel uncomfortable while rising, walking, or sitting. If there is an infection in the water-filled cyst, then pus formation is possible. Due to this, a patient may have a fever that may last longer. Often patients ask in OPD why these types of cysts occur.

WHAT IS THE PROBABLE CAUSE?

Usually, we see this type of cyst at 20-30 years of age. 

The reason may include:

  • Any injury or trauma
  • Some infections like Escherichia coli, Gonorrhea infection, or Chlamydial infection 

The patient often comes and asks whether stress is the reason for having Bartholin’s cyst. I would say no, stress doesn’t play any role in forming these cysts, but yes, it may lead to hormonal imbalance. But we can’t directly link it to the Bartholin cyst. 

Also, many times, patients ask if hormonal imbalance results in a Bartholin cyst? No, it’s not like that. 

Sexual Transmission 

Also, sometimes patients are in dilemma that Bartholin cyst is a sexually transmitted disease. No Bartholin cyst is not itself a sexually transmitted disease, but the causative bacteria like Chlamydia or Gonorrhea are sexually transmitted bacterial infections. But we can’t directly say that Bartholin cyst is sexually transmitted, then it’s not like that.

The second thing is that Bartholin cyst presents itself one side at a time. In a very rare case, we see its occurrence on both sides. Otherwise, it is seen on one side at a time.

HOW DO WE DIAGNOSE BARTHOLIN CYST?

Clinical examination performed by the doctor usually makes the diagnosis of this type of cyst. Depending upon the size of the cyst, symptoms, intensity of pain, and fever, a treatment plan is prepared. 

If you are above the age of 60 years, maybe the doctor would recommend a biopsy for a Bartholin cyst to rule out the possibility of cancerous growth. However, the occurrence of this type of cancer is less than 1 %. 

Sometimes this Bartholin cyst may rupture on its own. Small material oozes out as it enlarges and ruptures, including pus, bacteria, mucus, or blood. Often patients come with the symptoms of a ruptured cyst. 

WHAT ARE ITS TREATMENT PROCEDURES?

If we talk about its treatment, if the size of the cyst is very small, then we don’t require to do many things. Usually, the doctor advises the patients to have a Sitz bath at their house. The patient can take a Sitz bath by filling the tub with lukewarm water of approx 3-4 inches and sitting in it for 15-20 minutes. 

Apart from this, the doctor prescribes anti-inflammatory and painkillers to the patient to reduce the cyst symptoms. 

However, if it’s a large-sized cyst or pus formation or you are having intense pain, the doctor may advise you for surgery. In surgery, we do marsupialization these days. In this technique, we drain all the cyst contents and keep the cyst open so that secretions don’t get clogged, they come out, and the wound heals faster. 

But then also in 35-40% of cases, there is recurrent cyst formation, and the patient may require multiple siting of marsupialization. In this case, the doctor needs to remove the Bartholin gland, which is commonly called Bartholin gland excision. 

For this, the patients need to get hospitalized for one day at least. 

The procedure is performed under anesthesia, and technically it’s a little more complex than marsupialization. However, the procedure can be quickly done in patients with recurrent Bartholin cysts. 

HOW CAN WE PREVENT IT?

For its prevention, we must take care of vaginal hygiene and practice safe sex. If you have any issues, you may consult your doctor, get a diagnosis and start an early treatment under the supervision of the doctor.

What is Myopia or Nearsightedness? | What causes myopia

In this condition, the patients can see near objects very clearly, like reading or cooking, but they face difficulty seeing far-sighted things. This is called myopia. In this, the primary abnormality is enlargement of the eyeball, or for some reason, the rays of light don’t focus on the retina; instead, the light rays focus ahead of it. 

What is Myopia? There may be many reasons associated with it like a high power of the lens in the eye, steepness in the pupil, or enlarged eyeball, which is commonly known as axial myopia. So in these cases, the rays of light focus ahead of the retina rather than on the retina, especially in far-sighted objects. 

However, if we see near-sighted things, the light rays are focused on the retina. Hence we see clearly. 

What are the causes of Myopia? | Can myopia be cured

Myopia can affect any age; In children, it may start from childhood and may occur at the age of 6-7 years.  Due to abnormal eye growth, the rays of light focus ahead of the retina. So this problem primarily starts with eye growth.  

It may commence at the age of 6-7 years and  as the child grows, the eye enlarges, resulting in the severity of myopia. It grows maximum up to 15-16 years; after that, its growth recedes. Later it stabilizes by the age of 18-20 years. 

Hereditary could be the second reason for developing myopia. Those parents who have myopia or wear specs to see far-sighted things, their child is also at risk of wearing specs or myopia. The third reason is the occurrence of more indoor activity. As we have seen in the last two years, due to COVID, children have their online classes, and with less outdoor activity, they start developing myopia conditions. 

Everyone has to wear specs with minus numbers. But if we increase the outdoor activity, we can certainly stop the increase in number in specs.

How Common Is Myopia?

Myopia is prevalent in Asian countries like China or Japan, but it is less common in Western countries. For example, its prevalence in India is 30-32%, with the overall population.

Can Myopia Causes  Blindness?

Pathological myopia is one category of myopia in which the specs number is more than -5 or -6. So in these cases, the eyeball enlarge. The abnormal shape of the eyeball or because of the growth of the eye, apparently makes the retina and its inner layers thin. Due to this, some shortcoming starts to arise, like CNVN ( Cordial Neovascular Membrane ), in which there is the formation of some mass at the center of the retina where light rays focus. CNVN results in gradual loss of vision.

The second reason is the stretching of the retina due to enlargement of the eyeball. It leads to the formation of holes in the weakened areas of the retina. Holes or tears in the retina may lead to dislodging of the retina and causing loss of eyesight.

The third thing is due to the thin retina, the contrast ability of the eyes reduce. The image formed by the eye is dull or the image is with less contrast. These are common problems observed with myopia. 

Can We Treat Myopia?

See, we can’t treat myopia like we can’t make bigger eyes small. But yes, we can remove the number of specs. There are many ways to do so, and we have other videos on our Simplihealth channel. Laser surgeries or lens-based surgery are two  most common methods to remove specs. In laser, we integrate the number of the lens of specs in the eye itself, so we do not need to wear specs. We do lens-based surgeries for patients where the laser is not possible or unsafe; We use ICL or implantable columnar lens, which is a very popular technique. You can read in detail in our other videos.

Get Rid of Your Specs 

So we talk about removing specs. If your children have a minus number of specs, we don’t consider operation for removing specs in children.  So what can we do at this age? 

One proven and successful method is to increase the children’s outdoor activity. They should play outside for at least 1 hour  to 90 minutes so that eye muscles relax and eventually it stops the growth of numbers in specs.

The second thing is we must restrict the indoor activity of children. Suppose they have online classes or need to do some homework on devices. Keep them at some distance from the eyes. For example, use a laptop or tv screen rather than a phone.

There is some technology, though not available in India. First, some ortho lenses are available, like contact lenses, which we need to wear at night; they alter the eye shape and help restrict eye growth.  Next, we have Dems technology lenses available in specs and contact lenses; they modify the image formed by the eye, slowing down the development of an increasing number of lenses. But these technologies are not readily available in India because the government does not approve of them.  

As soon as they get approval in India, we will readily use them. But increasing outdoor activity is the most successful method.

Apart from this, we have low-dose medicines like atropine. So we need to put one drop in both eyes at night for six months. This also helps in reducing the growth of the number in specs.

Bottom line 

Hope this introduction about myopia is helpful to you. If your children are wearing specs of minus number, there is no need for panic. Give specs to your children and observe how the number is increasing concerning growth. If it is rising faster, you must consult your ophthalmologist; they will give the possible treatment to the children.  

Wearing specs is not being handicapped; instead, it acts as a mediator to focus light rays on the retina. Neither your child has weak eyes, nor is there an issue in the growth. It is just that they have different eye sizes. So there is no need to panic. Wear your specs and consult your doctor.

Today we will talk about your teeth and other oral hygiene measures. I will discuss this topic in three parts:

  • In the first part, we will focus on the oral hygiene of infants. 
  • In the second part, we will discuss oral measures needed for children in the age group 6-12.
  • The last part will be about children aged 12 years above and adults.

Oral hygiene in infants 

So let’s discuss part one, which is about infants and toddlers since a newborn baby is unaware of things like teeth. Oral hygiene of the children is the responsibility of parents. They need to either tell or teach their children that after every feed, especially after every feed, they should clean their gums so reflux should be formed in the child, as whenever he eats something, something goes inside his mouth and cleans it.

Since newborns do not have teeth, they must develop a mental or more appropriate neural reflex so that after each feed they are engaged in, something goes inside the mouth and cleans the gums. And this is primarily the responsibility of the parents, especially the lactating or feeding mothers. 

For this, you don’t have to do much like I have told you before. Take a muslin cloth, cut into three to four strips and boil it. Wrap the boiled muslin cloth on the finger and after every milk feed, clean their gums so that no food/milk particles adhere to the gums and will clean the bacteria if any are present. 

So in this way, we can take care of oral hygiene in infants. In addition, we can use some chemical aids such as Flouride drops. It’s nothing more like a chemical that acts on oral bacteria.

Oral measures needed for children in the age group 6-12

Then we come to children above six years as you must have observed that by the age of two to two and half years, every child has their milk teeth. During that period, parents must encourage their children to brush. We must make them brush twice a day. It should be done simultaneously on two teeth in a circular motion, five times clockwise and five times anti-clockwise from left to right and vice versa. 

In addition, we can rub their teeth with a fluoride drop solution readily available on the market. Then, the most critical age group is the age group of six-twelve years. By the age of six years, usually, children have their permanent first molar teeth. Their smiling or incisors teeth and lower incisors have erupted.

The children are in their mixed dentition period, in which they have few milk teeth and few permanent ones. So the parent needs to maintain and clean their teeth so that the children’s teeth are in good shape ahead of their life. So in this stage, we use toothpaste for children, which has 500 parts per million. I will repeat that 500 parts per million toothpaste are available in the market like Colgate or pepsodent, Chicco, ICPI, and many more. So whenever you buy toothpaste for the age group six-twelve years old, don’t buy an adult toothpaste; instead, select the one with 500 parts per million toothpaste. 

So how to use this type of toothpaste? 

The first thing is to use a junior brush with soft bristles and use paste equivalent to the size of a pea. Again we must brush in a circular motion from left to right extreme or vice versa. Similarly, do with the outer and inner side of teeth. Secondly, we can use pediatric mouthwash, which contains fluorides. And I will encourage you to add fluoride-based mouthwash to your children’s brushing regime. Finally, use fluoride-based toothpaste and mouthwash, let your child rinse with mouthwash at night, and avoid water so that the medicine effect lasts longer.

Oral measures for children above the age of 12 and adults 

By following the above measures, we can take care of the oral hygiene of the six-twelve  years of age group. But after the age of twelve years, as you must have seen, whenever we give any medicine to the children, we prefer liquid, whether it’s antibiotics or any pain killer, we prefer syrup as the concentration of medicine is less in syrup. But as the child grows, we start giving them tablets. Likewise, as the child develops, their requirements for toothpaste change. 

We use 1000 parts per million. We use 250 parts per million for infants and toddlers, followed by 500 parts per million for the age group of 6-12 years, and then we use 1000 parts per million for those above the age of 6 years. All the commercially available toothpaste have mentioned this in their ingredients. Irrespective of the brand of adult toothpaste, children in the age group above 12 years must use toothpaste with a fluoride content of 1000 parts per million.

Oral hygiene measures 

Children must brush before going to school. You may remove food particles by water rinse after lunch, but brushing should be the last thing at night before going to sleep. In addition, we do add mouthwash. I want to add a third thing which is dental Floss. It is nothing more than a wax thread that is either available in rolls or pre-cut strands or comes in a Y-shaped container. So these three things, especially at night; brush, mouthwash, and Floss, or brush, Floss, and mouthwash. 

You must follow this routine and must do Floss every night. In addition, and especially in children in this age group, we encourage them to brush in an up and down position so that if we are cleaning the upper layer of teeth by following up and down brushing, we are dragging food particles from up to bottom and vice versa in the lower set of teeth.

Likewise, we must similarly clean the innermost teeth. Keep in mind that we must clean their teeth carefully and efficiently. If we go by the book, the ideal time for cleaning teeth is 3-5 minutes. The children must brush for at least 3 minutes, especially at night. We must use Floss adequately. If we keep all these things in our minds, we can have as many sweets and fried food. Children may have anything like in a party they like to have coke. 

So if they take care of their teeth in the proper manner, they can have anything. So these are a few things that I would like you all to incorporate into your house to have a healthy smile for your children and trouble-free eating habits.

REASON 1:RETINAL DETACHMENT | Vision Loss Sudden

Vision loss sudden, Let’s see first, and foremost reason is detachment of the retina. Vision Loss Sudden, Many reasons might cause it. In some people, after cataract surgery, the chances of detachment of the retina are high. Sometimes during age-related changes, eyes have a jelly-type thing called vitreous. With the advancement of age, it starts melting, and after melting, it gets detached from the retina. So what happens when the vitreous is detached? It leads to retina detachment, and sometimes small holes are made in the retina. 

How does the patient recognize this? 

The patient feels like a curtain (retina) has come in front of the eye. This feeling comes to the patient as if a curtain has come on stage. For retina detachment treatment, one needs to visit an Ophthalmologist. After an eye check-up, when we see that there is detachment of the retina, so to rectify this, the operation is performed—also known as Vitrectomy. 

PROCEDURE OF VITRECTOMY | Vision Loss Sudden

In this, oil or gas is inserted in the eye to reposition the retina to its original place. Retinal detachment is a severe problem, and it should be treated urgently. And it is considered an emergency.

REASON 2: Retinal Vein Occlusion

A second important reason is the blockage of blood supply in the retina. This is known as occlusion or retinal vein occlusion. First, there is a clot in the veins of the eye that supplies blood to the retina.  And second is the retinal artery occlusion. In both cases, cells start dying slowly due to the stoppage of blood flow, leading to swelling in the retina.So in these cases too, suddenly there is a decrease in the patient’s eyesight which progresses more in 1-2 hours, and it gets essential to seek a doctor for treatment. If left untreated, there is a chance of Glaucoma, i.e., retina swelling and retinal detachment. This is known as Central vein occlusion or central retinal artery convulsion.In this doctor goes for an eye scan, and if there is swelling in the retina, there might be a need for injection or laser.

REASON 3: FLOATERS 

The third reason for the sudden loss of eyesight is blood in the retina. There is bleeding in the vitreous jelly, due to which light is unable to reach the retina. Hence the loss of eyesight.

This patient may see floaters or spots starting and roaming in the air because when there is blood on the retina, something is formed whose shadow falls on the retina. And if the blood increases in the eye, it might lead to complete loss of eyesight.

There are many reasons for this; however, in blood pressure and sugar patients, due to sugar, the retina is affected or may be due to high bp, the retina is affected.Due to this, new blood vessels are formed, which further leaks, and due to this leakage, blood comes in front of the retina leading to sudden loss of eyesight.

For this also, one must visit his doctor. It’s is observed for 2-3 months so that blood that has come on the retina settle on its own. If it doesn’t get settled, we need to operate it.

REASON 4:GLAUCOMA/Painful Loss of Vision

Next, we come to the fourth reason, which is glaucoma. In some glaucoma, there is a risk of sudden pressure, known as an acute angle-closure attack. In this, there is a pain with the loss of eyesight. It’s is called a painful loss of vision. There is a sudden rise of eye pressure by 40,50 maximum 60, typically less than 20.

COMPLICATION OF GLAUCOMA 

In this eyesight gets vivid, and in any light source, colorful bands are observed; one may feel pain in the eye, headache, and feeling of vomit.  This is also an emergency and maybe vision-threatening too. If not treated in 2- 3 days, there may be a complete loss of eyesight. In addition, eye nerves may get dry, so one must visit a doctor for this. This medicine is given first to reduce the pressure, followed by laser. Otherwise, the operation is performed to reduce the eye pressure.

REASON 5 : Optic Neuritis

The fifth reason is optic neuritis or swelling in the eyes. It is not common and usually occurs in 20-40 years of females. There are some diseases like multiple sclerosis or neuromyelitis optica, which are brain or spine-related but may affect eye nerves.This may happen after a viral infection, i.e., swelling in the eye. This is an autoimmune reaction, so it doesn’t affect directly but may show its effect on the eye. There is a sudden loss of eyesight, but within a few days, it is losing more. So one must visit the doctor who gives a steroid injection, which heals the swelling, and body tests are performed to find out the reason for this swelling. 

REASON 6: Optic Neuropathy | Vision Loss Sudden

The sixth reason is scheming optic neuropathy. This happens when one has a stroke, blood supply to the brain is stopped, or a heart attack in which there is a problem in heart supply. So there is a problem with the nerve which connects the heart to the eye. It’s just like the nerve attack. In this also, there is a sudden loss of vision. As such, there is no treatment for this, and it’s just that patients have problems like BP, Sugar, or high cholesterol; these things are controlled, and steadily eyesight comes again. It takes almost six months to recover from this, and after complete recovery, some damage is done, which is impossible to heal.

Cataract problems increase with advancing age. The actual God’s eye lens gets cloudy in this condition, just like frosting glasses. When an eye lens cannot reflect light, it causes blurry vision, or the incident light may produce glare which gives a blinding effect. Due to smaller pupils and cataract advancement, the patient starts losing sight. The cloudy lens is the first type of cataract. Cataract eye surgery & recovery time?

Advanced cataract surgery 

Nowadays, cataract operation can be performed at your convenience and takes significantly less time. Cataract surgery nowadays takes less than 10-15 minutes. In this procedure, we remove the cloudy lens and implant it with the artificial lens, called an intraocular lens. We don’t stitch or dress in this procedure, and we don’t require any abstinence. 

It is a painless procedure. We perform this surgery in drops, and the patient is free after 10-15 minutes. We must avoid splashes of water or any infection to the operated eye; however, we prescribe eye drops, and the patient heals fast. We can perform cataract surgery in two ways. One is Phacoemulsification which again is stitch less or otherwise we have a very advanced procedure, laser or flex surgery. To some, it is robotic surgery. Some call this robotic surgery. 

We achieve 90 percent of cataract healing through the touchless procedure. The laser melts the lens, and the liquefy makes the lens soft, and we remove the pieces and implant an artificial lens. With technological advancement, any lens is available. we need to make a port of 2.2-2.8mm to implant the artificial lens

Suitable lenses 

A few mini lenses require an incision of 1.2 to 1.8 mm with the help of a mini lens. We need to make a small port size which reduces the recovery time. And healing is much better. The third thing is that lenses can be unilocular, multifocal, or bifocal nowadays. Mini focal lenses improve farsightedness; however, we do require lenses for nearsighted lenses. 

The multifocal or bifocal lens gives almost near natural vision. We can’t mimic God-made eyes. It works for most people, and they are back to their routine. We require a small power lens in multifocal. Binocular, we must abstain from rubbing the eyes and splashing water. There are no precautions towards light intensity.

Benefits of a cataract surgery 

They may wear specs to avoid contact with dust in their eyes. We can do all our work without hesitation. Before surgery, we must consult our eye doctor for all these things; we must get our retina examined, consult our doctor about the suitable lens, and plan the surgery accordingly. These days this surgery has no connection with the weather. That was a complete myth that people used to avoid eye operations in the rainy or summer season. As you know, our life expectancy is increasing with every passing year. So the population of older people is also growing, as the percentage of elderly people is rising, so we have to deal with old age-related problems. As an orthopedic surgeon, the most common and most challenging situation that comes into our practice is fractures in elderly people.

How are the bones in elderly people, and what are the challenges we face? | Most Common Fracture Type In Elderly | Fracture Treatment In the Elderly People

When we talk about their bone health, especially when they are 55 or 60 in women and after the age of 60-65 in men, their bones get osteoporotic. Our experts mean bones become more fragile by osteoporosis, and their bone mass reduces. Eventually, they become soft. Due to softness, the bones break off easily. Hence, they are more fragile at a certain age and more delicate, so there is always a fear of breakage. So whenever there is slight trauma or an accident like foot slips, there is a fracture, especially around the hips. As you must have heard often, there was a little foot slip and a fracture in the hip, fracture in the wrist or spine, so they have fragile bones which break off easily.

The fourth challenge is that they always have one or another medical condition like a heart problem or sugar or a problem with blood pressure, making surgeries more challenging. Also, there are problems giving anesthesia to them. So there are a few challenges that we face in elderly people. But remember, our emphasis is on providing mobility to the old people who have fractures. So they should be able to stand or move on their own as soon as possible. Because if they keep lying in bed, their osteoporosis will get worse.

Do we need surgery for fractures in the elderly? | Fracture Treatment

So in all our practices, our emphasis, especially on elderly people who have a fracture, is to make them stand up early and use their body parts. This could be possible with good surgery. Like (our experts) told you before, our primary emphasis is that fractures heal as early as possible. The second thing is we prefer to join limbs in the normal position. So that the patients are in a mobile state and use their limbs as early as possible, so for this, we like to operate on these fractures. Few fractures don’t require hairline treatment. However, we do provide them with plasters. But we try to operate in most cases since the healing rate is very low in elderly people. So in this, as (our experts) told you before, giving anaesthesia is one challenge, so we need to control BP and sugar as much as possible before any surgery. 

TREATING FRACTURES IN ADULTS | Fracture Treatment

It is a multidisciplinary approach for elderly people. We must operate on them where all the disciplines are available like a cardiologist must be on standby, a reasonable anesthesiologist, and one must have a nice ICU unit. Even God forbid, all these specialists should be at hand if there is any complication. So elderly people (our experts) will always insist that if they are going through any trauma fracture surgery or any other surgery, they should select a hospital with all the disciplines, especially a cardiologist and intensive care unit. So this is the first challenge that we should operate in an atmosphere that caters to all these specialties. 

IMPLANTS USED FOR FRACTURES IN ELDERLY

Now we will talk about the implants. Now we have specialised orthopedic implants for trauma cases. As (our experts) told you before in a previous lecture, these are locking plates. If our bones are weak and we use standard plate screws, they will back out. So these days, we have special screws and plates for trauma and elderly people. These plates have a locking mechanism inside them, which locks the screw into the plate so that it doesn’t come out. So what happens with this? There are fewer chances of failure of implants. As (our experts) said, whenever there is a fracture, whether it’s in the spine or hip or hollow bone or bone with no strength in them. For this, we have artificial cement, which we put inside the bone and then implant or screw in the bone so that the hold of that implant increases.

Hip Fractures Among Older Adults

Then again, as (our experts) told us earlier, today we use nails. Like if there was a hip fracture earlier, then we put a plate, but nowadays, we have these specialised nails that we use, especially in hip fractures; there are long nails available. (our experts) emphasise this hip fracture because this is the most common fracture in elderly people. And if we don’t operate, the situation may worsen if left in bed for more than two months. 

Why is opting for surgery important in older patients?

(our experts) always advise my patients, even if it has a bit of high risk, please opt for surgery. Because of this beauty, we can make the patient walk on the very next day, and it is possible only with these new implants. So these are unique nails that we insert by giving a small incision around the hip and locking it. The purpose of these nails is to make the patient able to stand the very next day. Also, we do small incisions. The use of nails in hip fractures, especially for elderly people, is one of the most significant advancements for orthopedic trauma.

Bone graft

And another thing (our experts) would say is that God has given us something that works like Stepney is a bone graft. So even in young people whose fracture is old, more than three weeks old, or in elderly people where we know the bones are less intense and fracture won’t last with plates or nails, we take a bone from where we tie our belt; there is one bone there called an iliac crest. 

The procedure of bone graft 

From the iliac crest, we take tiny slivers of bones and put them with the fracture along with nails and plate, which enhances the ability and speed of bones to join. So these are called bone grafts, and we use them when we anticipate a fracture that heals slowly. And at three months old, fractures, so the bone graft is the big savior in these situations. So there is some advancement in this field of bone graft. We have some artificial bones that we mix with the natural bones and put in the fracture.

Dental health and oral hygiene in pregnant women 

When a woman is pregnant there is an upheaval of hormones in their bodies; some hormones increase, and some decrease. Due to this, changes occur in the whole body, and changes occur in the mouth as well. And these changes keep taking place till the child is born. And the most hormonal imbalance is seen when women are in the third trimester, which we say is from 6 to 9 months, and at this time, we see that women come to us with pain in the teeth, bleeding gums when they brush. 

They also complain that their teeth are shaking. All these changes are the aftereffect of hormonal changes occurring inside your body. 

At the same time, bacteria in the mouth become more active at this time because the food also has some specificity. They eat sour or consume more sweets. And eat all the time, and they cannot do the cleaning of their teeth properly. So that’s why I will talk to you today, especially about pregnant women walking in the 6th to 9th month of the month. He should take proper care of his teeth and gums. 

What to do? or How to clean your baby’s gums

Try your best to clean them thoroughly. And after brushing, try to clean your mouth by adding salt to hot water. Rinsing the mouth with salt water help in cleaning the food which might stick to your teeth. Your teeth will be excellent after the baby is born. If you clean your gums with dental floss, it will benefit you a lot and not spoil your teeth. You won’t even have blood in your gums. And by brushing correctly and brushing on time, the health of your mouth will be excellent. Teeth and gums will be healthy.

Oral hygiene instructions for newborn babies 

Now let’s talk about the newborn baby. When the baby is very young, it is a few months old; he takes most of his mother’s milk. And his mouth keeps moving all the time. Take a muslin cloth, take 3 to 4 long pieces, boil it and after feeding the children, wrap the muslin cloth on the finger and use the muslin cloth on the child’s gums. By doing the cleaning, the child will never forget that after eating anything, there is something that goes into my mouth and cleans my gums. And this habit of his becomes such that later you can turn it into brushing. If you have to brush three times a day, the child automatically understands. That’s something to do for your teeth. So you have to take great care of this thing. First, the newborn baby has gums on which teeth have not grown yet, has to be cleaned., And when its teeth start coming on the fourth month or six months, then you can clean it with a muslin cloth, especially at night time., when he sleeps after drinking mother’s milk. 

Changes to improve your baby’s oral health

Second, I will say that parents nowadays use a feeding bottle with a nipple in front, which is just a silicone nipple. I would encourage you to bottle feed your babies, not with a nipple, but with a sipper, so the milk doesn’t collect and go inside. If there is milk standing in the mouth of the child, then it will be that it will remain around the teeth. You will not be able to clean the baby’s teeth properly at night, and there will be cavity formation in the teeth. 

Avoid sweet to keep away dental cavity 

We have many children who have six missing front teeth. He says that it was fine at first, but it was over since it started giving bottle feed. Stress affects their mental health to a great extent. With intense assignments, work pressures, and deadlines, people usually neglect their diet and oral hygiene. And unemployed people are already in the phase of depression. All these situations have now become a significant issue in life. People with such problems come to us with the following presentations.

Due to irregular brushing, I would say again that people who do not regularly brush their teeth can have gums and tooth decay. Often patients come to us with improper oral hygiene, and they do not brush for 2-3 days. However, we advise people to brush their teeth at least three times a day, and these patients do not brush for three consecutive days.

Adenotonsillitis is a common condition in children where the tonsils swell, adnoids grow in size, followed by nose blockage. Since it affects our children, it is more important to discuss it. Because of this condition, there is a waste of school time. In addition, it impacts children’s social life and poses a severe effect on growth. 

All these things cause adenoid tonsil infection in children, so it’s an important topic. 

Why is this more common in children?

Because the immunity power in children is not yet fully developed, from 3 to 10-12 years of age. The maximum infection occurs at the age of  5-8 years. All these children are still building their immunity. When they visit school, they suffer from frequent community infections and are more likely to have viral or bacterial infections than other children. 

Symptoms 

Adenotonsillitis causes sore throats, fevers, frequent runny noses, and snoring. Children may lack sleep,  lack concentration or problems in studies, and may suffer from ear infections. Due to the large size of adenoids or tonsils, they suffer from frequent viral infections, affecting their school time. 

The frequency of frequent diseases in children is about 3 to 10 times in one year, which we must consider in the treatment. Suppose the child has such problems once or twice a year. In that case, we treat them medically; we give them an antibiotic course. Immunity boosters, and anti-allergic or nasal sprays are prescribed… To a large extent, the condition of such a child improves as time passes. But the children who have infections 5-7 times or every month, which we often see, cannot be treated with medicine only. They need some other way of treatment.

Causes of Adenotonsilitis 

Adenoid enlargement is an issue that should not be ignored. Adenoid enlargement occurs in children who have frequent allergies or viral infections. Adenoids are tissue present at the back of the nose, where it gives passage to the throat. There is an overgrowth of this tissue. However, a  bit of overgrowth is present in everyone, but in these children, it is enormous. Due to overgrowth,  their nose remains blocked, they snore at bedtime in particular, and nasal congestion, they cannot sleep correctly, hindering the child’s development. 

Also, we often see sleep apnea in these children. Due to inadequate sleep, their studies are affected, they lack concentration, and are behind in class. They develop ear infections. So the children of this category with persistent infection (5-7times in a year), hoarseness, fever, nasal congestion, complete blockage of the nose at night, loud snoring, and disturbed sleep–

There is the enlargement of adenoids in children with adenotonsillitis or chronic adenotonsillitis. We usually diagnose sleep apnea and tonsillitis. It is challenging to treat adenoids of grade three and four alone with the medicine.

Treatment of swollen tonsils (Adenotonsilitis )

Children with grade 3 or 4 tonsillitis need different treatments other than medicine. The essential diagnosis for this problem remains the clinical test. When we examine the children, we see inflammation in the enormous tonsils. They can be in grades 1- 4. In grade 4, they tend to touch each other. They become so big that there is no space left in between as they are touching each other. We usually see grade 3 or grade 4 tonsillitis in these children. On examination of the nose, adenoids are visible. To some extent, they are challenging to see in very young children, but they are easily visible in older children.

How is adenotonsilitis diagnosed? 

Otherwise, we can do an endoscopy or X-ray. X-ray lets us know the extent of blockage in the nasal canal, and there is no space for inhalation or swallowing food. It can also show intensity, i.e., the type of grade. Children with sleep apnea also have trouble sleeping and snoring and have more adenoids of level 3 or 4. Also, a complete blockage of a nasal canal can be confirmed by X-ray. 

There is no passage left for breathing; these children also have difficulty eating because of difficulty swallowing. They eat in tiny bits and take a lot of time to eat. They refuse to eat a lot of food, which affects their growth. We suggest surgery since no medicine can treat the enlarged tonsils with grade 4 or 3. Due to mechanical blockage, they are neither able to breathe nor able to eat correctly. Hence, surgery is much needed to open up this breathing passage. 

Now the question is, how do we do the surgery?

5 to 7 years from today, we perform without any instruments but with the help of cold equipment. By making an incision directly in it, accompanied by pain and blood, we suture the tonsils and later remove them. In the second therapy, we use cold instrument techniques. We insert a small cuvette inside the mouth to see all things on the screen. We remove adenoids under vision which improves the condition of the children.

Advanced endoscopic procedures 

But today, with the advent of new equipment, and endoscopic procedures, we insert an endoscope by a nose and melt the tonsils by observing them on screen with a laser. We use lasers for melting, and we can remove the whole tissue under sight.  Endoscopic procedures are bloodless procedures. This procedure is mainly known as plasma knife or laser therapy. Otherwise coblation is the most technically accurate word..

What is Coblation therapy?

All women face white-colored discharge at some point of time in their life. We treat white discharge many times, but it still keeps happening again. It can happen at any age. From young girls to pregnancy to menopause. The problem of this white discharge continues. Today we will discuss:

Understanding why white discharge happens

First, we must understand that white discharge can be a physiological means. What happens here?.

When ova are released from the ovum or follicle during the ovulation process, it is usually accompanied by transparent and sticky white discharge during a mid-cycle. So again, every woman goes through this phase, but the amount may differ for every woman.

It can be as little as a tablespoon. And if you don’t have a foul smell in the discharge. There is no stain on your inner clothes, no itching; this discharge is normal. We do not need any treatment, and as soon as the time of menstruation comes after ovulation, it may cause a little trouble. Its consistency changes. And then it becomes normal after menstruation. 

We talked about the normal discharge; if this discharge is smelling or changing its color, then we need to treat it. So for foul discharge, we should consult our gynecologist and do a health test. 

Risk factors of white discharge in female 

Patients often come to our OPD and say they get a very thick white discharge.

Commonly called  yeast infection, candidiasis, or fungal infection. The thick discharge is often more in the women who have diabetes or who are taking 21 days of pills, or who have taken many antibiotics. Lack of immunity or are immunocompromised in patients causes thick discharge due to yeast infection or candidiasis. 

Treatment of white discharge in female

For its treatment, we give simple antibiotics or antifungal medicine, by which white discharge problem resolves on its own. In many cases,  the color of this discharge becomes greenish or yellowish. There can be many reasons behind its color, such as infection of Trichomonas and Gonorrhea illness. The color of this discharge keeps changing under these illnesses. 

Causes of white discharge in female

Now, the patient often asks in the OPD why does white discharge happen?

The first reason is hygiene, which we always forget. Then there is some sexually transmitted disease or improper menstrual hygiene. So if you have a yellow or green discharge, we need to take antibiotics for it. Now there is another type of discharge that happens before or after menstruation. It often occurs when bleeding occurs after menstruation; then, it does not occur in the middle, there are some spots, and it is normal. So we don’t need to do anything about that. 

However, in some cases, there is spotting or discharge after periods. Let’s say you have a five-day cycle and have bleeding for five days, and then it stops, then again on day eight to nine, you have spots; this is called intermenstrual bleeding. We need to seek our doctor for intrauterine bleeding and get the proper treatment. There can be injury or could be polyps lying in the uterus. So there can be many reasons for the occurrence of intrauterine bleeding. But for this, we must get ourselves examined by the doctor.

What can we do from our side to minimize the discharge? 

Personal hygiene (White Discharge Treatment)

Whatever quantity of white discharge we have or it is repeatedly happening. First of all, I would say that we have to take care of some simple healthy measures. We don’t have to clean it with soap or water on every visit to the washroom. No, not at all, but we have to keep our private parts dry. Whether we use cotton for this or we have to keep it dry for some time. 

Vaginal hygiene 

Apart from this, whenever we clean it, we have to clean it from front to back, not from back to front, because by doing this, we are bringing the bacteria of our anal area into the vaginal area. This type of cleaning may cause bacterial infection in the vagina. So we always have to clean from front to back. 

Apart from this, we must increase the number of green vegetables and fruits in our diet. It gives us immunity, and we must also eat curd. It contains some good bacteria, which helps us to eliminate the discharge.

Apart from this, we have to take special care of menstrual hygiene; often, patients complain about an increase in discharge after menstruation. With time, it gets worse. So for this, I would like to say that we have to change sanitary pads after every 4 to 6 hours, and many patients spend the whole day in one sanitary pads, but this does not seem right. So we have to take special care of cleanliness. 

I would say that we should not use douches or any fragrance without asking our doctor. Apart from this, we need to intake ample water in our diet. Whether we are drinking juice or coconut or ordinary water, we have to keep an adequate quantity of Lassi, chach in our diet. Those who have diabetes should keep their sugar under control because due to uncontrolled sugar, white discharge increases significantly. 

Urinary tract infections may also increase the white release Plus we should not eat antibiotics unnecessarily. Taking antibiotics unnecessarily may alter the vaginal flora. Use antibiotics only when prescribed by the doctor… I am sure that if we use all these things, it will help reduce the frequent vaginal discharge.

Often you must have seen that people have pain in their teeth. The common symptoms of toothache are either it will hurt in one specific location, or it will go up to the forehead. The pain often gets worse when you lie down, and it goes straight to the forehead, hindering sleep.

Dental caries occur because of the accumulation of bacteria in which if you don’t clean your teeth or you have too much stress on sweets, or their consumption is too much. So it gets deposited in the tooth on which bacteria adhere since the teeth’s structure is of calcium, bacteria keep eating calcium slowly until one day the teeth break, resulting in a hole or cavity.

Tips To Prevent Dental Caries

So to prevent this, you must keep in mind a few things. We divide this into three types: first is primary prevention, which one must take care of prior to the infection so that you don’t have to live with this for your entire life. 

First, you must brush your teeth thrice, often people say twice. He recommends three times because we consume three major meals daily. And also, the method or brushing should be like one must brush after eating; like we brush in the morning but after having breakfast when we go to work or kids go to school, the food adheres to teeth all day long. So Dr. Bimal advises changing your brushing habit a little. Brush your teeth after eating. Even if you moisten the brush just to scrape off the food particles. According to him, You must brush after lunch, and the last thing at night to brush after dinner is essential. The brushing should be done for at least 3 -5 minutes. 

What Is Dental Flossing? 

Dr. Bimal introduces one more thing called dental floss. Which is nothing but a wax thread. Because you can brush your teeth upside down, how will you brush between the spaces of teeth as the bristles of the brush don’t reach there, so we use the thread of dental floss to clean it?  The second thing is diet, what type of food we intake is very important and What kind of food we consume, and at what time is also important. These are two-three things that you have to pay a lot of attention to. 

How are sweets bad for your teeth? 

Every dentist you will see will advise you to take fewer sweets or avoid them altogether. Dr. Bimal suggests having sweets intelligently. If you want to eat sweets, then eat before your main meal. If you’re going to go to any function, they have a sweet dish first, not last. Because later, when you have bread/roti, it goes down with it. The second thing is these days; all these snacks are available in packets; we have drinks or soft drinks, avoid them as much as possible. 

And if you want to have it, then have homemade fries or our basic old meals like samosa, pakode, or something like this so it won’t affect the teeth adversely. After that, I will tell you that you must gargle. And last but not least, there are certain chemicals that strengthen the other layer of the tooth. Like we use toothpaste these days. Patients often ask what to use for a toothache.

Which toothpaste to use? 

You visit the market, and you will see different types of toothpaste. There is no specific brand, but if you take any toothpaste with a high fluoride concentration, it makes your teeth stronger and strengthens the layer of the tooth. And at the same time gives protection from bacteria attacking the teeth.

Also, if you do mouthwash with fluoride content at a time, it adds to the effect. So few things have to be taken care of because dental carriers are such diseases that go very silently. But once there is a pain in a tooth, those who endure it only know the importance of their teeth. Dr. Bimal says that teeth are the first thing to carry food inside the body; they help enhance beauty and help in chewing the food.

Dr. Preeti: Hello to all the viewers. 

Harjot: We usually talk about the types of treatment of IVF. What are the new techniques that are available in the field of IVF about common people do not know? | IVF Center Near Me

Dr. Preeti Jindal: Harjot, Today, people are very much aware of IVF by google or any other means. Hence, they have a basic idea about IVF. Couples who find difficulty in conceiving are mindful of the term IVF. However, there are still many things in IVF that people are not aware for example, advanced forms of IVF: ICSI, IMSI, PGS, and ERA, so the couples who come to me are not aware of what type of IVF was done to them.

We should discuss everything in detail so that people know what they are going through and what doctors will do with these techniques.

Harjot: As you talked about ICSI, IMCI-like terms, it is often seen that couples go ahead with their treatment. IVF is a common term, but if we talk about ICSI, they know that they are doing ICSI procedure, but they should also know the procedure of ICSI is in detail and what kind of treatment is this.

Dr. Preeti: In ICSI like,  as we see on TV or in movies, we take one egg, and we implant it with the best sperm inside the egg. In simple IVF, we place the female egg and the sperm in a dish. This is the procedure for simple IVF. The best sperm naturally fertilize the egg to form the embryo. It is a cheap procedure, but it has a high failure rate because the sperm cannot diffuse inside the egg or the lining of the egg is very thick, and the sperm is unable to penetrate so the whole process will fail. So most good centers don’t do simple IVF; instead, they prefer ICSI. In ICSI, we magnify the sperm by 400 times, and we use the best sperm to implant the best egg after making a suitable identification. So this increases the chances of formation of the good quality embryo, and the second thing is when we take good sperm, the chance of abnormal fertilization is significantly less. So this process is called ISCI.

In comparison to IVF, ICSI is costly as this procedure requires specialized equipment and a trained embryologist who can work efficiently on the ICSI machine. All the good centers nowadays perform ICSI. ICSI has a good indication in the cases where the sperm factor is the main problem. Under these circumstances, they should opt for ICSI rather than simple IVF. Because if they have abnormal sperm or low count, then under 400 times magnification, we can select the best sperm. The second advantage of ICSI is when we are instilling the best sperm with the best egg; then there is an increase in the chance of formation of fertilized embryos. Now how is this helpful, since we can transfer only one or two embryos inside a woman at a time. 

So after seeking their permission, we freeze the rest of the embryos for that lady. Since the success rate of IVF is not 100%;  if we freeze them and, by any chance, the process fails for the first time, we can use the frozen embryos. This eases our emotional, mental, physical, and, most importantly, financial trauma. Because we can transfer the frozen embryo at less rate, these are some advantages of ICSI.

Harjot:  As you told me before, ICSI is a relatively costly treatment. So if we usually talk, since many patients come to you, I would like to know their perspective; do they value the cost of the treatment, or do they think they should opt for the proper treatment.

Dr. Preeti: Both things happen Harjot because when the couple visits us, they want to have a healthy baby. But at the same time, there is a cost factor in all these advanced treatments, so everyone can’t spend lavishly on this. So there is not much difference between IVF and ISCI. ISCI costs 20k-30k more as compared to IVF. But the problem is ICSI facility is not universally available. Only good centers have the facilities of ICSI. So the couples must know what they want. So when they visit any center, they should ask them about the type of IVF technique they will be using. Are you doing simple IVF or ICSI or using any more advanced technique. But new advanced methods certainly increase the success rate. 

Harjot: As you said it is more advanced and you said ICSI is already an advanced form of IVF. What is more advance than ICSI?

Dr. Preeti: Advancement depends on two things: if the sperm count is average, then which advanced technology we can use, and second is the advancement of IVF. If the sperm count is abnormal like there are many cases where men have abnormal sperm or have abnormal cont then we use a more advanced technique IMCI in which we can magnify the sperm 6000 times. And this process is called IMCI. The second thing is that if IVF and ISCI all went well, and the quality of egg and sperm was also good, the embryo was also good, but why the success rate is only 30-35% or 40% in young people. There are mainly two reasons for such kind of failure, like the embryo that was transferred was genetically abnormal,  the embryo seems fine, but the embryo’s genetic make-up is abnormal. So to detect an abnormality in the embryo we have PGS or Preimplantation Genetic Screening technique. The Touch clinic was the first in North Delhi to introduce this technique. The second reason might be that the lining of the uterus or the pregnancy bed was not ready at the time of embryo transfer. To detect this, we have the ERA technique Endometrial Receptivity Array, which tells about the time at which we should implant the embryo in the uterus so that it increases the chance of pregnancy. So these are all new techniques which we will discuss individually. 

Harjot: Since you’re talking about PGS, we would like to know more about it in detail. People are becoming aware of the treatment they are undergoing. So we are bringing all this information by this program so that everyone watching becomes aware of IVF and its advanced techniques. So I would like to know about PGS more in detail. What is the success rate of PGS in India?

 Dr. Preeti: First, we need to understand what PGS is? PGS is a Preimplantation Genetic Screening. Where does it come to play? It plays a significant role in improving the success rate of IVF because, as I told you before, sometimes the formation of an embryo is very good; however, when we transfer, it doesn’t result in pregnancy. The main reason behind this is the usage of genetically abnormal for transfer. Sometimes we transfer the embryo, which did result in pregnancy, but later women had an abortion in the first ten weeks. So all this usually occurs due to genetically abnormal embryos. But unfortunately, we can’t detect abnormalities in the embryo under a standard microscope. So to detect abnormality in the embryo, we have this new advanced technique PGS. In PGS, when the embryo is formed, with the help of a laser, a small hole is made in the embryo, and we extract one cell and study for its chromosomal setup. Hence we only transfer the genetically normal embryo.

So when we use PGS, it definitely increases the success rate of simple IVF by 15-20%. Now it comes to the mind that should everyone do PGS? Ideally, if there are no financial constraints, everyone should go for PGS. But since PGS is an advanced technique, we require special equipment, trained embryologists, and laser, which adds to expenses compared to simple IVF or ICSI. The cost of treatment is almost double. So we can use PGS where there is a recurrent failure in IVF or family who has an abnormal genetic baby before so we can detect that particular gene and can make sure that the next baby doesn’t have the same problem. 

The third thing is that we can see blood disorders like thalassemia or hemophilia in the embryo. So we should transfer only that embryo to females who don’t have genes for these abnormal diseases. So here, PGS plays an important role. One more important thing in PGS is that we don’t transfer the embryo instantly because we perform a biopsy of the embryo first. And till the biopsy report arrives which takes almost two weeks, so we freeze the embryos by unique technique. And when the report comes, in 30 or after 60 days, we transfer the embryos inside the women.

Harjot: Since you have been practicing it for the last 30 years and so many patients got healed, how much do you think these advanced techniques are brought awareness, and aced the treatment. 

Dr. Preeti: See, the technology always helps. When we talk about normal IVF. The success rate of simple IVF in which we place egg and sperm in a dish is not more than 20-25%, but when we perform ICSI, the success rate improves by 10%. In high sperm abnormality cases, we do IMSI. We were the first in the North to introduce this technique. We do IMCI in abnormal sperm count cases, increasing the success rate by 10%. Similarly, ERA and PGS increase the success rate by 15-20%. So when we talk about IVF, any technology that increases the success rate by a mere 5% so I think it is worth it.

Harjot: Since you talk about IMSI, we hear this term quite often, but we want to know about IMSI in detail. Since we spoke about ICSI and PGS, we must know about IMSI in detail. What is the significant difference between the two since their pronunciation is alike? We would like to know in detail.

Dr. Preeti:  In ICSI, we can magnify the sperm up to 400 times, but in IMSI, we can magnify the same sperm up to 6000 times. Nowadays, we observe in Punjab due to various reasons like pollution, especially drug usage resulting in low sperm count and increased abnormal sperm, so under these cases, IMSI plays an important role. In ICSI, we couldn’t find a good quality sperm under a standard microscope; however, here in IMSI, we magnify sperms upto 6000 times, so that we can easily pick a good quality sperm to fertilize the egg. Due to this reason, IMCI is not only helpful, and there is not much cost difference. But the problem is that IMSI technology is rarely available. 

As I already told you, we are the first in North India to introduce IMSI technology. However, it is present in some but still not readily available since it is a costly technology; it is not readily available. 

Harjot: The advanced techniques like IMSI and PGS are available at the Touch clinic, and whoever is watching our program, those who didn’t have success even after taking repeated IVF treatments, you need not feel sad; as all these techniques are available at Touch Clinic. Next, when you were talking about PGS we use the term ERA; we would like to know more about it. Since we talk about IVF, we have heard terms like ICSI and IMSI; however, we have little or no knowledge about PGS and ERA.

Dr. Preeti: IVF fails mainly because of two reasons either we have transferred an abnormal embryo, or during the transfer of the embryo, there were some problems in the formation of the uterus lining. To detect abnormal embryos, we have the technique PGS, but what can we do to detect the lining of the uterus. First, we can do an ultrasound to see the thickness of the lining, which everyone profoundly does. The thick lining doesn’t mean that it is ready to receive an embryo. In science, we have seen that the uterus lining has only 12 hours to accept the embryo, which might lead to a baby inside the body. And this 12 hr window varies in women. So we can detect this by ERA. In this, we perform Endometrial aspiration, a straightforward test, on the 17day of the cycle. Remember, this is the cycle where we are not doing IVF. We perform Endometrial aspiration, a straightforward test,  we use a very fine pipette, curette, or straw to extract the lining sample and send it for screening. We study 280 genes to know the exact time for a very high chance of pregnancy or successful embryo transfer. There is less chance of implantation failure, and the body will accept the baby. This is the procedure for ERA, and we were the first to introduce this technique in this region. We often perform  ERA on patients with multiple IVF failures and the couples who have opted for PGS, and it happens to be successful in many couples. Also, in a pocket-friendly and straightforward test. Those who are having multiple IVF failures; must go for this procedure. It is a good way forward. 

Harjot: Also, when I was reading about Touch clinic, I got this term PRP therapy. I couldn’t understand it by reading, but I may know by listening. So what is unique about PRP. Therapy.

Dr. Preeti: PRP stands for Platelet-Rich Plasma. Nowadays, I feel primarily young girls are more familiar with this term, not for IVF but for facial. Nowadays we do perform PRP facials. So we must understand what PRP therapy is. First, we take the patient’s three cc blood and process it with a special technique to generate platelet-rich factor or platelet-rich plasma. We make a platelet concentrate in the blood so that the amount of platelet is 3-5 times higher than the normal blood. Also, they have growth factors that we instill in the uterine cavity by a fine tube. It is a painless procedure. It is helpful in cases where patients have some problems in the formation of the uterine lining during IVF or are having recurrent miscarriages. Secondly, we instill these on the faces of young girls for face rejuvenation, more commonly known as Vampire facials. Since it has a growth factor, it helps remove dead skin and regenerate new skin. PRP plays a vital role in many cases, like even we use this in our IUI cycle to achieve success. These are some latest technologies that help increase the success rate of IVF or IUI pregnancies.

Harjot: Earlier also thorough other videos and from Dr. Jindal’s informational talk, we are providing your information about the latest techniques and advancements, Dr. Preeti Jindal, every time she comes she tells us in what way medical science is progressing in the direction of available techniques. Today also we gained a lot of information from her.

I would also like to know, however, nowadays that many people are aware of IVF and other advanced things. But how much time do patients take to accept these advanced therapies.? Like they should have taken this test. 

Dr. Preeti: People are knowledgeable. They don’t make much time if we make them understand well. If we involve them in our treatment, they know what theory we are doing. Like in our center, we spend at least 30 minutes with every patient. We have a separate counselor to explain all these techniques to them, and we also give leaflets to read in English, Hindi, and  Punjabi. After that, we call them again and ask for their queries. So people are very wise these days. They know what is good for them, so they tell us that this technique will suit us better. We aim to involve the patients rather than saying they need this treatment. We don’t work in that manner because the most important thing to achieve pregnancy is to work coincides with the couples since no one knows which technique is best for them since there is no 100% success in any procedure. Our aim is for the couples themselves to see which technique will be more helpful to them, and without wasting their time and hard-earned money with the least intervention, we need to help them. Those who come to us, we make them understand all the procedures carefully and help us give treatment to them. They are part of the decision-making.

What is IVF or test-tube baby?

In this, we use injection to produce eggs in a lady’s ovary. We aim to produce 10-15 eggs, and when these eggs retain their proper size, i.e 17-20 mm, we remove the eggs vaginally from the female’s body under sedation. Later we fertilize the eggs with the partner sperm, and we incubate them outside in the incubator. After day 3 or 5, there is embryo formation. Then, we transfer 1 or 2 embryos into the female body and freeze the remaining embryo for later use after seeking permission. This whole process is called IVF or test tube baby. 

IVF is of many types. The simplest among them is that we place the eggs and sperm of the partner in the same dish and allow them to fertilize naturally. 

Next step of IVF treatment 

The following advanced technique is ICSI which is also the next step of IVF. It is rather an advanced form of IVF. In this, with the aid of a special microscope, we fuse the sperms with eggs. By this method, we can magnify the sperm up to 200-300 times, and it helps in picking the right sperm and egg. We fertilize them. Its success rate is more than usual IVF by 10-15%. Another advanced technique is IMSI. It is mainly for those people who have abnormal or low sperm count. In this method, we can magnify the sperm up to 6000 times and use the best sperm for fertilization with the egg. 

The following advanced technique Is PGS or Preimplantation Genetic Screening. We perform the embryo’s biopsy by laser, and we do special screening on one cell to determine any abnormality. We choose genetically normal embryos only. Another important and helpful thing in IVF is ERA or Endometrial Receptivity Analysis. It tells the right time to transfer the embryo inside the uterus by analyzing the inner lining of the uterus. We have seen that the uterus lining is open only for 12 hours to receive the embryo, and it varies differently in every female. With the help of ERA and PGS, we can increase the success rate of IVF by 15-20%. 

Other things help improve the success rate of IVF, like Platelet-rich Plasma or PRP therapy. We can use PRP therapy in cases where the uterus has a faulty lining, or the lining formation is incorrect. Alternative therapies have also been found to be helpful, like acupuncture, Acupressure, or massage therapies. There are a few medicines like vitamin D and antioxidants which can improve the success rate of IVF.

How are fractures in children managed?

Fracture management in children, As you all know, children are pretty mischievous. They play here and there, and sometimes it gets complicated to save them from any injury. But as a parent, if we think this type of injury requires doctors’ treatment or it seems they might have a fracture, we must seek doctors’ advice. Children less than 15 years old have different bone structures compared to adults. Children’s bones are relatively soft.

Like a branch of a tree, if we bend it, it breaks. And the bones in children are like the soft green ones, which do not break completely when bent. So children’s bones are very soft and pliable. And they are half cartilage and half bones. Since they are growing, they have more soft tissue cartilage, and the maximum we miss such cartilage Injuries in children. 

So they are harmful because of certain other factors, but they are less dangerous because of other factors. So today, we will talk about pediatric fracture and how we deal with it.

Children often come with pain and swelling with a history of falls while playing. And very young can’t tell exactly where it hurts. Under these circumstances, we have to interpret their actions like we can diagnose where they are swelling and pain. If it’s on the leg, they cannot put weight on it, and if it is in the hands, there will be swelling, and obviously, the child won’t be able to use that area. So the first thing we advise the parents to do is X-Ray. If the fracture in the cartilaginous part is not visible in X-Ray, mostly known as epiphyseal injury. 

We usually miss these injuries, so to avoid missing these injuries, we often perform the XRay of the opposite arm because the growing end of children often seems like a fracture and is often overlooked. To avoid this, we do complete the X-Ray of the opposite arm. In a few cases where we can’t see the fracture, we opt for an MRI. But mainly, we can interpret the kind of fracture by X-Ray. And like I said, sometimes we need to do an X-Ray of both sides.

WHAT TYPE OF FRACTURE OCCURS IN CHILDREN?

One is buckle fracture with no crack; however, it bends slightly. Another one is greenstick fracture, in which it bends and barely breaks, so it’s an incomplete fracture. There are other known fractures. There are some known areas among children, like when they fall, they fall on their palm, so there is a fracture in the forearm which could be a green stick or complete fracture. Again a prevalent fracture occurs around the elbow, more commonly called a supracondylar fracture. The child falls with the outstretched hand with pain or swelling around the elbow. In the leg, fractures are common in the leg bone, thigh bone, or middle ankle leg, and if there is a significant fall or accident, then thigh bones may crack too. So we mainly diagnose these fractures by X-Ray; in the rare case, we opt for MRI. 

Fracture Management in Children: HOW DO WE MANAGE CHILDREN’S FRACTURES?

The good thing about children’s fractures is that they heal efficiently and fast. This is because children have a high capacity for bone healing. In adults, when we heal fractures, our prime aim is to connect the bones either by plaster, nails, or plates. In children, if the bone is lying distant, it still heals, so healing is faster and is much quicker. This factor quite favors the children. That’s why most fractures can be managed with simple plaster unless it is a buckle or incomplete fracture. Then, we put plaster in the same positions to have less or no pain and swelling.

Within 3 to 4 weeks and in some cases in two weeks, the bone heals, and we don’t require any further treatment except for plaster. In a few cases where fractures are displaced a lot, as I told you, their bones heal faster, but one thing you should keep in mind is that even if they lie distant, they will recover but if we allow them to heal in the wrong position it could result in deformity. 

Like if the fracture in the leg, there can be an overriding of bones, bones will heal; however, a similar thing is not possible in adults, but there will be shortening of legs, and of course parents nowadays are very anxious, and in children also it is certainly not acceptable. In some cases, if it heals in different angulations, the bone might recover in the crook position, which is also not permissible by the parents. 

But I would like to tell you that some sort of angulation is acceptable because there is remodelling in bones as they grow. The crooked part automatically heals in the correct position to some extent at right angles. They remodelled themselves. This angulation is not visible in the thigh bones, where there is muscle mass. 

In a few cases, it might generate a cosmetic problem, but it doesn’t cause many problems in children. The main problem is when we miss injuries like epiphyseal injuries, which usually occur around the joint made of half cartilaginous and bones. If we can diagnose it correctly, sometimes we may need an MRI. Since it’s a joint fracture, we need to set it in its normal position. We don’t require an incision, but we fix it by putting some wires with the minor surgery. We can pull these wires after 3 -4 weeks. So in a few cases, we need to operate, especially when there is epiphyseal injury or injury around the joint or a displaced fracture; bones are lying in an unacceptable position, and we operate under these conditions. 

Most children can do very well with the plaster. However, there is a word of caution that some children can’t express their issues with plasters, like if it’s tight or they are having some problems, so as an orthopedic surgeon and as a parent, you have to be very alert. An adult may tell you that the plaster got wet, painful, or tight. However, in children, we need to be alert that there are no blue fingers, the plaster is not tight, and sometimes children put coins or something else inside the plaster to itch, so we must take care of all these things since they are different than in adults. We can open plaster after 3-4 maximum six weeks, start the exercises, and injuries in children heal very well.

The only problem is that we miss them, but we diagnose them appropriately. You should be very careful and immediately rush children to the doctor because the bones heal very fast, and if they recover in the wrong position, it limits the option for doctors. So friends, look after your children, be alert towards children, and whenever you feel the need, please go to a nearby orthopedic surgeon and get treated accordingly.

Heroic snoring is a severe problem, and they usually have sleep apnea. Most patients with significant daytime sleepiness with heroic snoring suffer from obstructive sleep apnea.

Mild snoring

In mild snoring patients, patients snore lightly, and little sound is produced, which may cause a disturbance. These patients may not have apnea or daytime sleepiness but have social and marital problems and don’t like to go out with others or outside society. We need to address these patients too. 

Causes: why do you snore? 

The most common reason for mild snoring is that they might have some allergy causing nose blockage. A second reason could be structural problems like having polyps in the nose, deviated nasal bone,  turbinate hypertrophy, or being slightly obese. And they have extra tissues in throats or enlarged tonsils or tongues or a thick palate. All these problems may contribute to mild snoring. 

For medical causes like allergies or polyps, we can treat them by giving anti-allergies treatment or decongestant therapy. With the help of steroid spray, we can clear the nasal blockage. and it may improve their snoring. Also, we ask patients to lose weight, engage in some activity, improve their muscle tone, do exercise, and the patients heal by following these things. If they don’t get better, we treat them surgically as no medical treatment is available.

Patients with mild snoring often don’t visit us because they don’t consider this a problem; they think it is part of life and not a disease. But those who are worried have issues affecting their social and marital life; they come to us for treatment, and we address and treat them adequately. 

How does heroic snoring affect your life? 

Patients with heroic snoring snore enough to make noise in the adjoining room, or those who sleep alone because their spouse can’t sleep with them; we need to rule out apnea in their history. Mostly they have sleep apnea with significant daytime sleepiness. In addition, 80 % of heroic snoring patients are moderately obese or are slightly overweight. Finally, most heroic snoring patients have a history of smoking and alcoholics with the intake of tranquilizers and sleep medicines, so all these have a higher chance of heroic snoring. 

Especially in obese people or those who have a short neck, or chin indrawn, or have different type of face or chin structure; so all these patients come under the severe category; they require serious treatment.

Treatment: how can you treat your snoring? 

It is impossible to treat them medically with medicines. These patients suffer significant daytime sleepiness and loss of work hours. They are prone to have accidents while driving, and they have a considerable life modification which has happened because of various things. We need to diagnose these patients by examining and ruling out the structural problem; we need to perform endoscopy of the nose and throat. We need to check their body weight and BMI since they are all obese. We need to take their lifestyle history and rule out alcohol intake, tranquilizers, and smoking. 

Sleep study | How to Stop Snoring Naturally

The most important thing in heroic snoring is the study of sleep. Sleep study or polysomnography is the gold standard investigation as it tells us about the cause of severity and gives the idea of a prospective treatment. We do sleep study overnight. We study all the sleep parameters and their oxygen and carbon dioxide levels, their breathing pattern in sleep, how they breathe at a particular position, what position they snore more or EEG, and whether their brain is asleep or not. So we need to study all these twelve parameters for a comprehensive sleep study, and after we do all the analysis and advise the patient accordingly. 

There are different categories of sleep study. During a sleep study, if the snoring causes mild or moderate sleep apnea, one feels shortness of breath. So firstly we advise these patients to reduce weight. We request them to come to their weight according to their BMI. They must do gym or exercise and keep diet precautions to maintain weight.

Structural abnormality and CPAP machine 

For those who have a structural abnormality, we need to correct them surgically. Those who can’t do all this and are suffering a lot need therapy, so we advise them to CPAP. These people breathe with masks at night. It is a mechanical mask, and there is a pump inside that delivers air under pressure; these patients adapt within 3-5 days. CPAP users sink their breathing with the mask and get comfortable with it.CPAP is one and best treatment for everybody. But the problem is everyone is not willing to use CPAP and is not mentally prepared; they find it difficult to use. 

Surgical treatment 

This disease is severe anyways, so we do surgery on these patients. It is a relatively more extensive surgery in which we work on all three things; nose, throat, and voice box. It is a major surgery in which we need to advance the jaws to make space for airways. Gross surgery is a lengthy procedure, and most people can’t tolerate it. Therefore, weight reduction, exercise, and CPAP therapy are the best treatment for morbid snoring or heroic snoring, or morbid obesity. 

Natural tips to stop snoring

  • We must avoid alcohol, smoking, and sleeping pills, and we need to maintain our sleep hygiene.
  • We need to sleep in one position. 
  • Don’t sleep upright; rest on the side or inverted
  • They need to keep the head in a higher place so that the throat tissue does not fall into the breathing area to maintain sleep hygiene.
  •  One must have a light dinner and exercise regularly. 
  • And the patient must also avoid driving to prevent any accidents.

Patients are fine during the day; as soon as they lie at night, there is a blockage in the nose. This can happen on one as well as both sides.  Patients also complain of dryness of the mouth snoring, and their sleep remains uncomfortable. 

Common causes of nasal blockage 

The postural blockage is pretty apparent, but we discuss some important reasons. The most obvious reason is that these patients have some bone deviation or deformity, so they accentuate while lying down. Because there is blood pooling at night, it swells, causing nasal blockage due to the lying position.

  • Further little swellings affect their bone structure, or their structural problem escalates the nasal block. 
  • The second reason is that when people drink more or do any recreational drugs, they also suffer from postural nose blockage. Those people snore a lot, and this is a common problem. 
  • The third cause is that maybe some people take blood pressure medicines, which also contain vasodilators, due to which the blood vessels swell, resulting in swelling of nasal mass. Vasodilation medicines could also be a reason. 
  • The fourth cause is that some people are very allergic. And in allergies, many people are allergic to dust mites, which can cause allergies. The dust mites are primarily present in the bed or the mattress, and as they lie, they come in contact with the dust mites resulting in swelling in the nose.

They may sneeze a lot during the night and have nose blockage for the maximum time. So these are some important reasons for postural nose blockage that patients have during the night.

What are the treatment for nasal bloackage? 

Most patients cure themselves with nasal drops at home. Over the counter, they take drops from chemists and use nasal sprays for months. However, prolonged use of nasal decongestants is more harmful. Their blockage also increases, and their need for nasal drops increases even more. And then we are not able to treat them efficiently.

If we see treatment for these patients, those who are allergic patients, those who are allergic to dust mites, they can take anti-nasal, or by spraying the steroid nasal spray, there can be healing in postural blockage. 

For those who complain about nose blockage due to alcohol, the apparent remedy is that they need to avoid drinking. Those who have structural problems like bone deviation or the turbinate are too big, or they have allergic changes in their nose, or there are polyps;  then they can be treated smoothly by surgery. They should consult their doctor, as we can cure them through surgical procedures. Those suffering from blood pressure have to visit their cardiologist, so changing the medicine cures this postural blockage. 

You can add a local topical decongestant, but there is a limit; you cannot add more than 5 to 7 days. If you cannot visit the doctor within 5 to 7 days, you can use it, but after that, you have to take treatment from the doctor and do not put nose drops. Long-term use of nasal drops can cause severe damage to your nose.

Can Squint Eye Treatment Without Surgery? 

We can correct some squints even without operation, like some children who wear specs of very high numbers. This condition is called accommodative esotropia, i.e., they have skewness due to not wearing specs. Children who wear specs of very high numbers like plus 4 or 5; try to put pressure to see clearly, and because of the pressure, their eyes go inwards. 

This is called accommodative esotropia, for which we ask to put atropine, an ointment in the eyes. After that, we check their eyes, and accordingly, we ask the children to wear the specs with the updated number. In this way, we can correct this type of skewness. As these children grow up, the number of their specs decreases. This skewness also starts to heal independently. 

Another type of skewness, intermediate divergent squint, or it happens intermediary, and we can be cured without an operation. When the patient is in his thoughts or sleepy or tired, one eye turns towards the outside of the ear. 

It is recommended some special convergent exercises to strengthen muscles which helps in the inward movement of the eye so that this squint goes away. So in such patients, if they wear specs with a high number or a lack of vision, we correct their eyesight by giving specs, which increases their control over the eyes. And this skewness comes under control. So these are two types of squints that we can cure without operation.  

Adults may develop squinted eyes due to diabetes, thyroid diseases, brain tumours, and strokes. In addition, due to stressful life, children and adults develop eye fatigue problems, and their vision gets blurred. However, one can opt for options like specific exercises that can help squint the eyes. Another effective treatment is the use of prism glasses. Prism glasses can help in reducing blurred vision. Squint Eye Surgery, There is a 90 – 95% success rate in the most common type of squint involving no muscle damage. But if the child is a little younger, then squint can come back after 5-10 years. But the reoccurrence is only in 5-10% of the cases. 

Now let’s talk about those patients whose eye muscles have stopped working or have become worse for some reason or those who have structural problems. Under these situations, it is hard to determine the success rate of the squint operation. It depends on which muscles of the patient are working and the patient’s expectations. 

This type of skewness in which no muscles are working or the eyeball cannot rotate in one direction, then we can straighten the eye, but if the muscles that are not moving or are not working, we can’t heal it. So it depends on what kind of squint it is. But in 90-95% of the cases, skewness operation is effective.

Is Squint Eye Surgery Painful?

Like any operation, squint also causes pain in operation, but to ease the pain, we give injections to numb the eye in adults, and its effect lasts for 1 hour to 90 minutes so that the patient doesn’t feel pain during an operation. 

In contrast, the child is entirely sedated for the operation so that they also do not have pain. There is some degree of pain on the first day after the operation, for which we give painkillers so that the pain does not become a big problem.  

At what age the squint operation should be done?

We have made a more detailed video on squint in children; you can see that too, but let me tell you that squint operation depends on what kind of squirt the child is bearing, which further determines the age at which we can do the operation. If the child’s eye is turned inwards or towards the nose, we also call it esotropia; In such patients, it is better to have the operation as soon as possible because, in esotropic patients, vision development is not possible. 

So if we delay the operation, it becomes challenging to improve the eyesight. Another type of skewness in which the eyes are on the outside is called exotropia or divergent squint. At whatever age the child can get a reliable test, we can operate on them at that age. 

Otherwise, there is no such thing that the operation can be done when they grow up. As I told you, esotropia is a danger to the eyes. So we can do the operation even at one year of age. And children who have exotropia can wait for up to 3-4 years of age, but if they remain untreated, they will lack in developing the ability of 3D or in-depth perception, so we have to operate early. 

How long does a squint operation take?

It depends on how many muscles we need to operate. It takes about 30- 75 minutes. It takes 30 minutes to operate on a single muscle, and if we need to work on  3 to 4 muscles, the operation might take one hour to 75 minutes.

Do we need glasses after a squint operation?

If you were using glasses before the operation, you have to wear glasses after the procedure. Whether you are a young child or an adult, wearing glasses is necessary. The number of specs can change. After the operation, we check the number of specs after a month or in 3-4 weeks, and we give new specs to the patient.  

Is patching treatment needed after the operation?

It depends on the type of skewness and eyesight. For example, suppose the child is young, and there is the development of lazy eyes or amblyopia, on which the doctor  has made another video. In that case, such patients may still need patching after the operation to improve vision.

What are labor pains? When you feel the right labor pain?

Labor Pains Symptoms, You should rush to the hospital. We often think of minor aches during the last month of pregnancy. We call these regular Braxton Hick contractions. People often confuse false pain with labor pain. But when we have labor pains, we have to understand that it hurts very badly. During labor pains, minor aches occur frequently and with increased intensity. Like earlier, the pain was coming every 10 minutes, which lasted for 10-15 seconds, but during labor pain, this pain gradually intensified; as if earlier it was coming every 10 to 15 minutes, now they are coming every 5 minutes. They will start coming in minutes, and the duration of pain increases from 10 to 20 seconds to  20 to 30 seconds. They will start coming more quickly. 

Like we say that every 5 minutes, you will begin to have pain. And at the same time, their duration becomes worse, with pain lasting for 30 to 40 seconds. And these pains do not go away from doing anything. In this, the woman cannot do her daily work, and changing any position does not help ease the pain. Instead, their duration and intensity increase, then we should understand that this is actual true labor pain. And at this time, we need to rush to the hospital for admission. 

The second thing is that whenever we talk about delivery, I always explain the following  four things to my patients, 

  1. You will start feeling labor pain.
  2. You may burst your water bag or leak water (your water would break).
  3. The third thing is a decrease in fetal movement or reduced baby activity.
  4. And the last thing is bleeding. 

If you see any of these symptoms, you must contact your doctor. 

When should you go to the hospital? 

Now we have discussed labor pains and what is the right time for the hospital admission. 90-95% of the patients have labor pain before delivery. However, in only 5 to 10 cases, the leakage of water commonly called a burst of water, occurs, followed by labor pains. Patients often get confused when there is too much leaking. Whenever there is a leak or rupture in the bag, a transparent liquid like urine oozes out. But even in the last month of pregnancy, patients may have the condition of frequent urination.

Also, the baby puts pressure on the bladder, causing a slight leakage of urine. So patients get confused to a great extent, whether it is urine or leakage of the sac. See, there will be a kind of smell in the urine, and its quantity will also be less, and if you go to the washroom, it will reduce the leakage. But, when the bag bursts, we get so much flow that the clothes get wet. And it goes on continuously, and it doesn’t stop even after urinating.

The third thing is what we call, decrease in the baby’s activity in the last three months of pregnancy. The doctor recommends keeping track of fetal movements especially in the last two months. If the fetal movements are more than ten, they are entirely normal. But anytime you feel that the baby’s activity is decreasing, you should visit your gynecologist. 

Sometimes we think less movement, but everything seems fine when your doctor checks with an ultrasound. Sometimes the child must be in their sleep cycle, or the mother has not taken a proper diet. So these two things may lead to reduced fetal activity. But sometimes, it can also signify that the child is under stress. So sometimes the baby is facing some problem, or if the water around the baby has become less, this may also reduce fetal movement. Whenever something like this happens, you must immediately consult your gynecologist. 

The fourth thing is the occurrence of bleeding. During the last trimester of the pregnancy, the labor pain onsets, and the release of cervical mucus plug leads to a bloodstain. So because one doesn’t have proper bleeding, there is a little bit of blood stained discharge. If anything like this happens, then I would say that you go and meet your doctor immediately. We have understood our signs of having pain, leakage of water, lack of activity, or any bleeding. 

According to the doctor’s opinion, you should directly go to your doctor and prepare yourself for the hospital admission.

Uterus removal means removal of the uterus by surgery. Hysterectomy has become a general surgery and is the second most common surgery after the cesarian. 

What conditions do we need to remove the uterus? 

Uterus removal surgery is our last resort. When we are not benefiting from medicine or any other procedure, I would say that we give this surgery of uterus removal option to the patient. Patients often come to OPD and complain that they have been discharging very fluently, so please remove the uterus or have children; our family is complete, so remove the uterus so that we don’t face any further problems. We don’t think the same about the rest of our body parts, their lungs. 

  • Do we remove them, or we have intestines? 
  • Shall we remove them too? 
  • So why do we think the same about the uterus? 

CANCER | Uterus Removal Surgery

The first reason is cancer. If you have uterus cancer, cervix cancer, or cancer in ovaries or fallopian tubes, there is seriously a need to remove the uterus. It is an extensive surgery and takes time because we need to do it in detail. 

FIBROID

Apart from this, the following most common reason for uterus removal is fibroids.  We need to understand this thing about fibroids. Some fibroids are very small in size l, and they are not cause any problem, so we don’t need to remove the uterus in these cases. However, sometimes these fibroids increase in size and may reach up to 15-16cms, or they may grow up to 8-10 cm in size. So in the case of a large fibroid, we need to remove the uterus. Though fibroids are often small, the patients are bleeding a lot. They visit us frequently with 2-3gm of hemoglobin, and even after giving medicine and hormone-related medicine. If their symptoms are not improving; there is a frequent drop in hemoglobin.Under these cases, the doctor advises surgeries. So that after surgery, you can remove the uterus. 

ADENOMYOSIS

There is another prevalent condition which is called adenomyosis. In this, as the period comes, the bleeding is getting collected somewhere in the muscle layer of the uterus. In everyday language, we say this to the patient: there is swelling in the uterus. Since the bleeding doesn’t find any outlet increasing the size of the uterus, periods start coming early from the times there is heavy bleeding. And one may feel a lot of pain during the duration of periods as much as they can’t do their household routine work. So when there is a condition like this, when we have tried medicines and hormone coated copper-T, but still the patients feels no relief, then we advise uterus removal or hysterectomy to the patient.

ENDOMETRIOSIS

Another common condition is endometriosis, where the innermost layer of the uterus, the endometrium, starts growing in different parts of the stomach. So when these lining is growing, they stick to each other, causing pain. We have good medicine, but few patients don’t react to it. So we advise them for surgery. Finally, there is a need for uterus removal under a 3rd degree UV prolapse, in which the uterus slides down.

There is another condition where the innermost layer of the uterus, is endometrium, gets thick, called endometrial hyperplasia. In cases where the condition is going towards a pre-cancer stage, they also need to remove the uterus surgically. Suppose there is some change taking it to the precancerous stage called atypia. In this case, the doctor chooses to remove the uterus or follow any hormonal treatment how do you want to stratify the risk. So in these cases also, we advise uterus removal. Usually, in minor conditions like endometrial polyps, if we have any discharge or there is some infection of the cervix, in these cases, we don’t advise removing the uterus.  And even if we have to remove it, there are all types of treatment available, even for cancer. We operate with a telescope  in which patients recover faster and are better cosmetically.
